术中舒适护理对人工膝关节置换术患者术后疼痛及膝关节功能的影响

Influence of intraoperative comfort nursing on postoperative pain and knee joint function of patients having received artificial knee replacement

目的探讨术中舒适护理对人工膝关节置换术患者术后疼痛及膝关节功能的影响。方法选择2012年6月~2014年6月在我院行人工全膝关节置换患者100例为研究对象,分为两组,对照组行常规护理,干预组在此基础上于术中着重实施舒适护理,比较两组术后1 h、术后1 d、术后3 d的疼痛评分变化情况;以及两组患者的膝关节功能优良率。结果随着时间延长,干预组与对照组的VAS评分逐渐降低,且干预组术后1 d、术后3 d的VAS评分显著低于对照组（P〈0.05）。干预组的优良率90.0%,显著高于对照组的优良率72.0%,两组膝关节功能比较,差异具有统计学意义（χ2=4.638,P〈0.05）。两组术后SAS评分分别较入院时显著降低（P〈0.05）,干预组术后SAS评分（31.7±3.3）分,对照组术后SAS评分（38.5±5.6）分,组间比较,差异具有统计学意义（P〈0.05）。结论术中对人工全膝关节置换术患者行舒适护理具有较好的效果,可以帮助患者缓解心理压力而配合治疗,同时有利于减轻术后疼痛,对于改善膝关节功能、促进病情的康复具有较大的促进作用。

Objective To investigate the influence of intraoperative comfort nursing on the postoperative pain and knee joint function of the patients having received artificial knee replacement. Methods One hundred patients who received artificial total knee replacement in our hospital from June 2012 to June 2014 were selected as the study subjects, were divided into two groups. The control group was given conventional nursing and the intervention group was given comfort nursing particularly on the basis of the control group. The pain score changes of the two groups 1 hour, 1 day and 3days after surgery were compared. The excellent and good rates of knee joint function of the two groups were compared.Results As time went by, the VAS scores of the intervention group and the control group decreased gradually, and the postoperative 1 day and 3 days the VAS scores of the intervention group were significantly lower than those of the control group, with significant differences（P〈0.05）. The excellent and good rate of the intervention group was 90.0%, which was significantly higher than the 72.0% of the control group; the two groups were significantly different in the knee joint function（χ2=4.638, P〈0.05）. After surgery, the SAS scores of both groups reduced significantly compared to those at hospital admission, with significant differences（P〈0.05）; the postoperative SAS score of the intervention group was（31.7±3.3） points and that of the control group was（38.5±5.6） points, with significant difference（P〈0.05）. Conclusion The application of intraoperative comfort nursing in the patients receiving artificial total knee replacement shows favorable effects, can help relieve the patients＇ psychological stress and thus increase compliance, and is also conducive to relieving postoperative pain, which plays a promoting role in improving the knee joint function and accelerating rehabilitation.
