1例接种乙肝疫苗后出现5次呼吸暂停的案例分析

目的分析从化市1例接种乙型肝炎疫苗(Hepatitis B Vaccine,Hep B)后发生呼吸循环系统症状并出现5次呼吸暂停的疑似预防接种异常反应(Adverse Events Following Immunization,AEFI)病例的原因,以评价Hep B安全接种工作,指导今后疫苗安全接种工作,防止预防接种纠纷事件的发生。方法对现场搜集病例的临床资料、预防接种资料进行整理和描述性分析。结果此案例是1例接种Hep B后偶合病毒性脑炎病例,不属于预防接种异常反应,虽与疫苗接种无直接关联,但在偶合反应中不排除有诱发或加重病症的关联;因在接种前家长曾告知患儿有不适现象,最后事件以接种单位给予一定经济补助终结。结论婴幼儿是一个特殊群体,这个时期也是接种疫苗的重要时期,在排除家族遗传和自身体质先天疾患外,家长日常的观察与关注接种前的健康问询工作是预防和减少接种疫苗出现偶发症的重要手段;预前检查及健康问询内容记录是缓解和减少预防接种偶合症引发的医疗纠纷事件发生的有效佐证材料。
