脂联素、瘦素、抵抗素与妊娠糖尿病胰岛素抵抗相关性分析 被引量:6

Correlation analysis of adiponectin,leptin,resistin and gestational diabetes mellitus insulin resistance

摘要 目的分析妊娠糖尿病患者脂联素(adiponectin,ADP)、瘦素(leptin,LEP)、抵抗素与胰岛素抵抗(insulin resistance,IR)的关系。方法选择某院2014-2015年妊娠晚期糖尿病患者60例为观察组,并选取同期健康妊娠期者60例为对照组。采用ELISA法检测ADP、LEP及抵抗素水平,葡萄糖氧化酶法测定空腹血糖(fasting blood glucose,FBP),放射免疫法测定空腹胰岛素(fasting insulin,FIns)水平,比较两组以上相关指标,并分析与胰岛素抵抗指数(Insulin resistance index,IRI)相关性。结果与对照组比较,观察组的TG、TC、LDL-C均明显升高,而HDL-C降低,两组比较差异均有统计学意义(t=12.36、11.46、9.85、8.77,P<0.05)。与对照组比较,观察组ADP水平较低,而LEP及抵抗素、FBP、Fins及IRI均高于对照组,两组比较差异均有统计学意义(t=15.32、14.77、9.02、6.11、7.55、6.31,P<0.05)。经相关性分析可得出,LEP、抵抗素、ADP与IRI具有相关性,其中LEP与IRI呈正相关(r=0.867,P<0.01),LEP、抵抗素与IRI呈负相关(r=-0.807、-0.788,P<0.01)。结论妊娠糖尿病患者瘦素、抵抗素水平升高,而脂联素水平下降,瘦素、抵抗素与胰岛素抵抗具有相关性,是胰岛素抵抗独立危险因子,脂联素是保护因子。 Objective To analyze the relationship between pregnancy diabetes adiponectin (ADP), leptin (LEP), serum resistin and insulin resistance (IR) . Methods From 2014 to 2015, 60 patients with late gestational diabetes were observed, and the healthy pregnancy at the same time were 60 cases as the control group. Using ELISA method to detect ADP, leptin and resistin levels and glucose oxidase method determina- tion of fasting blood glucose (FBP) and radioimmunoassay determination of the level of fasting insulin (fins), comparing more than two groups of related indicators and analysis correlation with IRI. Results Compared with the control group, the TC, LDL - C and TG of the observation group were significantly higher, while the HDL - C was lower, and the difference between the 2 groups was statistically significant ( t = 12. 36, 11.46, 9.85, 8.77, P 〈 0.05 ) . And compared to the control group, the observation group ADP level is low, and LEP and resistin, FBP, fins and IRI were higher than the control group, 2 group differences were statis- tically significant (t = 15. 32, 14. 77, 9. 02, 6. 11, 7.55, 6. 31, P 〈 0. 05) . The correlation analysis can be drawn, LEP, resistin, ADP, and IRI is associated, which LEP and IRI were positively related (r =0. 867, P 〈 0.01), leptin, resistin and IRI was negative correlation (r = 0. 807, - 0. 788, P 〈O. O1 ) . Conclusions Pregnancy diabetes leptin and resistin levels increase, and decrease of the level of ser- um adiponectin, leptin, resistin and insulin resistance is associated, adiponectin is the independent risk fac-tor for insulin resistance.
