64排螺旋CT血管造影在颅内动脉瘤诊断中的应用 被引量:9

摘要 目的:研究64排螺旋CT血管造影在颅内动脉瘤诊断中的应用。材料与方法:我院2012年1月-2014年1月收治了112例颅内动脉瘤患者,选取其中依据64排螺旋CTA结果进行手术治疗的患者96例。64排螺旋CTA检查:扫描层厚0.625 mm,后处理技术包括应用多层面法(multip lane reconstruction,MPR)、容积显示法(volume rendering,VR)和最大密度投影法(maximum intensity projection,MIP)进行三维图像重建。并对照手术结果评价64排螺旋CTA在颅内动脉瘤诊断中的应用价值。结果:64排螺旋CTA共发现96例104个动脉瘤,其中88例为单发动脉瘤,8例为2个动脉瘤。动脉瘤瘤体最小直径1.9 mm,最大直径37.4 mm,平均15.3 mm。所有患者均依据64排螺旋CTA的结果及SSD模拟的手术入路影像完成手术,在术中均能顺利找到并夹闭动脉瘤,共夹闭动脉瘤104个。CTA能清晰显示动脉瘤大小、位置、形态及与周围结构的关系。手术中发现动脉瘤所在的位置与CTA检查结果一致,符合率达到100%。结论:64排螺旋CTA对颅内动脉瘤的诊断有较高的准确性,对制定动脉瘤的治疗方案有重要的指导价值,适合在临床应用中得到推广。 Purpose: To study the 64-slice spiral CT angiography in the diagnosis of intracranial aneurysms. Materi- als and Methods: Select Ninety six patients from One hundred and twelve patients who were in our hospital from January 2012 to February 2014 with intracranial aneurysms, which is surgeried only based on the results of 64-slice spiral, and the examina- tion of 64-slice spiral CTA : slice thickness was 0. 625 mm, post-processing technologies, including application of muhip lane reconstruction method, volume rendering method and maximum intensity projection were three-dimensional image reconstruc- tion. The value of CTA in diagnosis and operation of intracranial aneurysm s wasevaluated with the results of surgery. Results : A total of 104 aneurysms were detected by 64-slice spiral CTA in 96 patients, including 8g cases of solitary aneurysm, eight cases of two aneurysms. Artery aneurysm minimum diameter was 1.9 mm, maximum diameter was 37.4 mm, and an average was 15.3 ram. All patients were based on the results of 64-slice spiral CTA and SSD images simulated surgical approach to complete the surgery, and 64-slice spiral CTA can successfully show the location and aneurysm clipping, which were clipping a total of 104 aneurysms during surgery. 64-slice spiral CTA can clearly show the relationship between the aneurysm size, lo- cation, shape and surrounding structures. The coincidence of aneurysms confirmed by surgery and those detected by 64-slice spiral CTA was 100%. Conclusion: The accuracy o f 64-slice spiral CTA is higher in the diagnosis of in tracranial aneurysm, and it has. important guiding value for the development of aneurysm treatment programs. It is suitable for popularized in clinical applications.
