螺旋CT对86例骨关节损伤的诊断分析及临床价值 被引量:3

摘要 目的:探讨螺旋CT在骨关节损伤诊断中的临床应用价值.材料与方法:选取2014年9月~2015年3月我院门诊收治的86例骨关节损伤患者作为研究对象,所有患者均先行传统的X线平片检查,再行螺旋CT扫描.比较两种检查方法的结果.结果:86例骨关节损伤患者中,有盆骨骨折31例,膝关节骨折18例,肩关节骨折12例,踝关节骨折7例,肘关节骨折6例,肩关节脱位6例,腕关节脱位3例,髋关节脱位3例.经过MPR、SSD及VR重建,发现12处关节发生脱位和半脱位,38处有碎骨块,24处有关节内积血或周围软组织血肿.螺旋CT扫描的2DCT(86.0%)、SSD (94.2%)及MPR(98.8%)诊断准确率均显著优于X平片(65.1%).差异均有统计学意义.结论:螺旋CT扫描及其重建后的处理功能可以准确提供骨关节损伤的空间关系信息,对骨关节损伤的诊断有重要临床应用价值,为临床医师制定治疗方案提供参考依据. Purpose: to study the clinical application value of spiral CT in the diagnosis of joint injury. Materials and Methods: select in September 2014 March 2015 of 86 joint injury patients admitted in our hospital outpatient service as the research object, all the patients were first conventional X-ray plain film examination, spiral CT scanning. The result of comparing two methods. Results: 86 cases of joint injury patients, there are 31 cases of pelvic fractures, fractures of the knee joint 18 cases, 12 cases of shoulder joint fracture, 7 cases of ankle fractures, 6 cases of elbow fracture, 6 cases of shoulder joint dislocation, wrist joint dislocation in 3 cases, 3 cases of the hip joint dislocation. After MPR, SSD, and VR reconstruc- tion, found 12 joint dislocation and subluxation, 38 has broken bones, hemorrhage or soft tissue around the hematoma within 24 in the relevant section. Spiral CT scan of 2 DCT (86. 0% ), SSD and MPR (94. 2% ) (98.8%), the diagnostic accuracy were significantly superior to X-ray plain film (65. 1% ). Differences were statistically significant. Conclusion: spiral CT scan and reconstruction processing functions can accurately provide joint injury of spatial relation information, has important clinical value to the diagnosis of joint injury, and provides reference for clinicians to make treatment plan.
