我院门诊处方点评及用药合理性分析

Our-Outpatient Prescription Reviews and Rationality Analysis of Drug Use

目的：分析我院门诊处方不合理的原因及存在的问题,为临床合理用药提供参考。方法：采用回顾性调查,随机抽取2013年1月~6月我院门诊处方共3 000张,对处方进行点评及对不合理处方进行分析。结果：不合理处方占调查处方的比例为9.83%,其中用法用量不适宜最多占3.3%,其次为未写诊断或诊断书写不全占1.6%。结论：用法用量不适宜与未写诊断或诊断书写不全为我院不合理用药的主要表现形式,创新合理用药举措尤为必要。

Objective： To analyze the problems and reasons of the irrational outpatient prescriptions, and to provide reference for clinical rational drug use.Methods： A retrospective study was conducted on 3 000 randomly selected outpatient prescriptions in our hospital from January to June 2013, to comment on those prescriptions and then analyze the irrational ones of among them.Results： Unreasonable prescriptions accounted for the proportion of survey prescription 9.83%,which is not suitable dosage up to 3.3%, followed by the lack of diagnosis or incomplete diagnosis which accounted for 1.6%.Conclusion： Improper dosage, lack of diagnosis together with incomplete diagnosis were the main manifestations of the irrational drug use in our hospital, therefore, the innovation of the rational drug use is particularly necessary.
