社区团队顾问制对2型糖尿病管理效果分析

Analysis of the Effect of “Community Team Consultant System” Model in Community Management of Type 2 Diabetes Mellitus

目的:分析社区团队顾问制对2型糖尿病管理效果。方法:将上海市闵行区颛桥社区的400例血糖控制不佳的糖尿病患者随机分为对照组200例、干预组200例。对照组按目前普通门诊调控血糖。干预组由团队顾问制专家协助普通门诊强化干预管理。比较两组干预前后的情况。结果:干预后,干预组较对照组在血糖达标率及医院满意度等方面均有明显提升,差异有统计学意义(P<0.05),诊疗费用无显著变化。结论:"团队顾问制"社区糖尿病管理模式在改善糖尿病血糖达标率,医院满意度等方面效果显著,值得推广应用。

Objective： To Analysis of the effect of community team consultant on type 2 diabetes mellitus.Methods：The 400 cases of poor glycemic control in diabetic patients were randomly divided into two groups.The control group according to the current general outpatient blood sugar regulation. Assisted by a team of consultants in the intervention group made intensive intervention specialist general outpatient management.Results：After the intervention,the intervention group than the control group had improved significantly in terms of blood glucose and hospital satisfaction rate,the difference was statistically significant （P〈0.05）. Conelusion：＂Team Advisor＂ system of community diabetes management model can improve glycemic compliance rate, improve hospital satisfaction, should be widely applied.
