二维超声斑点追踪技术评价高血压心力衰竭患者左心室功能 被引量:4

摘要 目的二维应变成像技术是一项新的评估心肌功能的超声心动图技术。运用二维应变方式研究高血压心力衰竭患者的左心室收缩功能,并通过多普勒应变测量进行验证。方法选取2013年10月—2014年12月在本院住院的81例NYHA分级为Ⅰ级到Ⅳ级的高血压患者,23例高血压患者为NYHAⅠ级组,21例高血压患者为NYHAⅡ级组,20例高血压患者为NYHAⅢ级组,17例高血压患者为NYHA IV级组。20例健康者作为对照组。通过Scheffe事后比较检验和χ2检验对连续的和明确的变量分别进行分析。用Levene检验评估方差齐性,P〈0.05为差异有统计学意义。结果 NYHAⅠ级患者的基底室间隔节段、NYHAⅡ级患者的基底和中部室间隔以及基底侧面节段、NYHAⅢ级和Ⅳ级患者所有节段的纵向应变降低。径向和周向应变在NYHAⅢ级和Ⅳ级患者中有所降低。在可接受范围[特殊节段纵向应变为(0.61%~1.92%)±(2.38%~2.92%),径向应变为(4.98%±5.26%)]内,二维技术和多普勒应变技术仍存在一致性。结论高血压患者中:(1)左心室收缩功能在NYHAⅠ级到Ⅳ级患者中逐渐减低,异常情况开始于基底室间隔。(2)左心室径向和周向收缩受损出现在NYHAⅢ级和Ⅳ级患者中。(3)二维应变测量可定量左心室收缩功能。
