老年2型糖尿病患者生命质量的影响因素分析

Quality of Life and Its Influential Factors Among Old Patients Diagnosed with Type 2 Diabetes Mellitus

目的:评价老年2型糖尿病患者生命质量及其影响因素。方法:采用SF-36量表评价1 740名老年2型糖尿病患者的生命质量,同时调查患者人口社会学特征、生活习惯、疾病情况等信息;采用多元线性回归分析生命质量影响因素。结果:生命质量综合评分平均为71.48±19.86分以及生理机能、生理职能、躯体疼痛、一般健康状况、精力、社会功能、情感职能、精神健康等8个方面(PF、RP、BP、GH、VT、SF、RE、MH),生理领域、心理领域等(PCS、MCS)2个领域的得分均低于中国糖尿病常模(P<0.05);与中国一般人群比较,除MH外的7个方面以及2个领域(PCS、MCS)的得分均低于中国一般人群(P<0.05)。多元线性回归多因素分析结果表明,PCS评分模型中运动习惯(B=-5.458)、合并其他慢性病(B=8.443)、年龄(B=-4.395)、家庭人均月收入(B=-5.131)、婚况(B=5.895)等5个因素纳入方程,该模型的整体拟合情况决定系数R2为0.120;MCS评分模型中运动习惯(B=-4.998)、家庭人均月收入(B=-4.837)、婚况(B=6.041)、年龄(B=-2.596)、腰围(B=-3.042)和合并其他慢性病(B=3.397)等6个因素纳入方程,该模型的整体拟合情况决定系数R2为0.086。结论:老年2型糖尿病患者生命质量受损严重,运动习惯、婚况、年龄、家庭收入和是否合并其他慢性病是影响生命质量的因素。

[Objective] To evaluate the quality of life （QOL） of old patients with 2 diabetes mellitus （T2DM） and to explore its in- fluencing factors. [Method] SF-36 questionnaire was used to evaluate the QOL of 1 740 old patients with T2DM, and sociological character- istics, life habits, chronic diseased-related conditions were also collected. Multiple linear regression analysis was used to choose the influen- cing factors among QOL [Result] The average QOL score of all the patients was 71.48 ± 19. 86, lower than Chinese diabetes norm in PF, RP, BP, GH, VT, SF, RE, MH, PCS and MCS respectively （ P 〈 0. 05 ） . Results of multiple linear regression multivariate analysis showed that 5 factors entered the equation of PCS score model, including exercise habits （ B = - 5.458） , combined with other chronic dis- eases （ B = 8. 443 ）, age （ B = - 4. 395）, family income （ B = - 5. 131 ）, marital status （ B = 5. 895）, the overall fit of the model case of the coefficient of determination R^2 was O. 120. Totally 6 factors entered the equation of MCS score model, including exercise habits （ B = - 4. 998）, family income （ B = - 4. 837） , marital status （ B = 6. 041 ）, age （ B = - 2. 596）, waist circumference （ B = - 3. 042） , and combined with other chronic diseases （ B = 3. 397 ）, the overall fit of the model case of the coefficient of determination R^2 was 0. 086 （ P 〈 0. 05 ） . [Conclusion] The QOL of old patients with type 2 diabetes mellitus was damaged universally and exercise habits, marital status, age, family income and whether or not combined with other chronic diseases were the factors affecting the QOL .
