辨证分型治疗慢性胆囊炎随机平行对照研究

Differential Diagnosis and Treatment of Chronic Cholecystitis Parallel Randomized Controlled Study

[目的]观察辨证分型治疗慢性胆囊炎疗效。[方法]使用随机平行对照方法,将80例住院患者按就诊顺序号法简单随机分为两组。对照组40例50%10m L硫酸镁,3次/d,口服。治疗组40例辨证分型:肝胆湿热(利湿清肝、消热利胆,半夏、枳实、栀子各10g,薏苡仁30g,大黄、丹皮、柴胡、黄岑、白芍各15g),1剂/d,水煎200m L,早晚口服;脾胃气虚(补气健脾,甘草炙、人参各10g,白术、半夏、陈皮各15g,茯苓20g,木香5g),1剂/d,水煎200m L,早晚口服;肝胆郁结(解郁疏肝利胆,白芍20g,香附、川芎、枳壳、柴胡各15g,川楝子10g),1剂/d,水煎200m L,早晚口服。脾虚肝郁(通泄补脾,山药、茯苓、白术各20g,白芍、陈皮各15g,甘草、砂仁、防风各10g,木香5g),1剂/d,水煎200m L,早晚口服;脾肾阳虚(脾肾温补,党参、茯苓各20g,干姜、白术各15g,甘草炙、肉桂、枳壳各10g,木香5g),1剂/d,水煎200m L,早晚口服。连续治疗14d为1疗程。观测临床症状、不良反应。治疗1疗程,判定疗效。[结果]治疗组痊愈25例,有效12例,无效3例,总有效率92.50%。对照组痊愈11例,有效13例,无效16例,总有效率60.00%。治疗组疗效优于对照组(P<0.01)。[结论]辨证分型治疗慢性胆囊炎疗效满意,无严重不良反应,值得推广。

[Objective]To observe the treatment of chronic cholecystitis syndrome type effect.[Method]uses random parallel control method,80 cases of hospitalized patients with treatment order number method is simple and randomly divided into two groups.40 cases in the control group 50% 10 m L magnesium sulfate,3 times /d,orally.Treatment group,40 patients Syndromes：gallbladder damp heat（dampness Liver,gallbladder elimination heat,Banxia,Zhishi,Zhizi 10 g,Yiyiren 30 g,Dahuang,Danpi,Chaihu,Huangcen,Baishao 15g）,1 a/d,decoction 200 m L,sooner or later orally;spleen Qi deficiency（Spleen,Gancaozhi,Chaihu 10 g,Baizhu,Banxia,Chenpi 15 g,Fuling 20 g,Muxiang 5g）,1 agents /d,decoction 200 m L,sooner or later orally;bile stagnation（stagnation Shuganlidan,Baishao 20 g,Xiangfu,Chuanxiong,Zhike,Chaihu 15 g,Chuanjizi 10g）,1 a/d,decoction 200 m L,sooner or later orally.Spleen Liver depression（through vent spleen,Shanyao,Fuling,Baizhu 20 g,Baishao,Chenpi 15 g,Gancao,Sharen,Fangfeng 10 g,Muxiang 5g）,1 a/d,decoction 200 m L,sooner or later orally;spleen deficiency（spleen warming,Dangshen,Fuling 20 g,Ganjiang,Baizhu 15 g,Gancao,Rougui,Zhike 10 g,Muxiang 5g）,1 a/d,decoction 200 m L,sooner or later orally.14 d for a course of continuous treatment.Observation of clinical symptoms,adverse reactions.Course of treatment 1,determining efficacy.[Results]The cure 25 cases,effective in 12 cases,3 cases,the total efficiency of 92.50%.Cured 11 cases in the control group,effective in 13 cases,16 cases,the total efficiency of 60.00%.Treatment group than in the control group（P〈0.01）.[Conclusion]Differential Diagnosis and Treatment of chronic cholecystitis satisfactory effect,no side effects,it is worth promoting.
