清肺调血汤合金匮肾气丸联合西药治疗慢性阻塞性肺疾病稳定期随机平行对照研究

A randomized Controlled Trial of Chronic Obstructive Pulmonary Disease in Stable Parallel Tiaoxue Qingfei Decoction and Jingkuishenqi Pill combined with Western Medicine

[目的]观察清肺调血汤合金匮肾气丸联合西药治疗慢性阻塞性肺疾病稳定期疗效。[方法]使用随机平行对照方法,将64例住院患者按抛硬币法简单随机分为两组。对照组32例沙美特罗替卡松粉,1吸/次,1-2次/d。治疗组32例清肺调血汤合金匮肾气丸（桂枝25g,制附子15g,牡丹皮39g,茯苓36g,山萸肉15g,山药36g,熟地黄40g,蝉蜕21g,薏苡仁40g,麻黄炙15g,白茅根20g,芦根15g,虎杖10g,侧柏叶、桃仁各15g,杏仁10g;畏寒肢冷加干姜;下肢浮肿者加葶苈子、牛膝;纳差加麦芽炒、神曲;大便稀加山药、当归;眠差加合欢皮、酸枣仁炒;小便不利加龙骨煅、金樱子）,1剂/d,水煎300m L,早晚分服;西药治疗同对照组。连续治疗7d为1疗程。观测临床症状、FEV1、FVC、FEV1/预计值、不良反应。连续治疗2疗程,判定疗效。[结果]治疗组痊愈15例,显效8例,有效8例,无效1例,总有效率96.32%。对照组痊愈3例,显效4例,有效4例,无效21例,总有效率33.25%。治疗组疗效优于对照组（P〈0.01）。肺功能指标两组均有改善（P〈0.01）,治疗组改善优于对照组（P〈0.05）。生活质量评分两组均有改善（P〈0.01）,治疗组改善优于对照组（P〈0.01）。[结论]清肺调血汤合金匮肾气丸联合西药治疗慢性阻塞性肺疾病稳定期疗效满意,无严重不良反应,值得推广。

[Objective]To observe the Tiaoxue Qingfei decoction and Jingkuishenqi pill combined with Western medicine treatment of chronic obstructive pulmonary disease in stable efficacy.[Methods]Randomized parallel controlled method,64 cases of hospitalized patients by flipping a coin method is simple randomized into two groups.The control group of 32 patients salmeterol and fluticasone propionate powder,1 suction/time,1 to 2 times/d.Treatment group of 32 patients treated by Jinkui Shenqi Qingfei decoction and transfer of blood（Guizhi 25 g,Zhifuzi 15,Mudanpi 39 g,Fuling 36 g,Shanyurou 15 g,Shanyao 36 g,Shudihaung 40 g,Chantui 21 g,Yiyiren 40 g,Zhimahuang 15 g,Baimaogen 20 g,Lugen 15 g,Huzhang 10 g,Cebaiye,Taoren 15 g,Xingren 10g;chills add Ganjiang;lower extremity edema plus Tinglizi,Niuxi;anorexia plus Chaomaiya,Shenqu;stool Shanyao,Danggui;plus poor sleep Hehuanpi,Chaosuanzaoren;urine is not american Duanlonggu,Jinyingzi）,a/d,decoction 300 m L,morning and evening hours;Western medicine with the control group.7d for a course of continuous treatment.Observation of clinical symptoms,FEV1,FVC,FEV1/predicted,and adverse reactions.Continuous treatment of 2 courses,to determine efficacy.[Results]The cure 15 cases,8 cases were markedly effective in 8 cases,one case,the total efficiency of 96.32%.The control group recovered three cases,4 cases were markedly effective in 4 cases,21 cases,the total efficiency of 33.25%.Treatment group than the control group（P〈0.01）.Lung function improved in both groups（P〈0.01）,the treatment group than the control group improved（P〈0.05）.Improved quality of life scores in both groups（P〈0.01）,the treatment group than the control group improved（P〈0.01）.[Conclusion]Tiaoxue Qingfei decoction and transfer of blood Jingkuishenqi pill combined with western medicine treatment of chronic obstructive pulmonary disease in stable results were satisfactory,no side effects,worthy of promotion.
