凉血五根汤联合西药治疗血热妄行型过敏性紫癜随机平行对照研究

[目的]观察凉血五根汤联合西药治疗血热妄行型过敏性紫癜疗效。[方法]使用随机平行对照方法,将318例门诊及住院患者按掷骰子法简单随机分为两组。预防和控制感染,明确上呼吸道感染或感冒时选用抗生素或抗病毒药物;注意休息,防止过劳,避免服用可能引起过敏的药物及食物如葱蒜、辣椒、酒类、海鲜等。对照组150例依巴斯汀,10mg/次,1次/d;盐酸西替利嗪滴剂,1m L/次,1次/d;上呼吸或其他感染用头孢丙烯分散片,0.5g/次,1次/d;雷公藤多甙,1mg/kg,3次/d;维生素C,0.1g/次,1次/d;芦丁,20mg/次,3次/d;变态反应严重予复方甘草酸苷,50mg/次,3次/d;腹痛重予阿托品,0.3mg/次,3次/d;关节肿痛予优布芬,50mg/次,3次/d;呕血便血予西咪替丁,0.2g/次,2次/d。治疗组168例凉血五根汤(白茅根30g,紫草根15g,茜草根20g,生地30g,地榆20g,槐花15g;关节型加秦艽、独活、木瓜;腹型加白芍、玄胡、甘草;肾型加大蓟、小蓟、仙鹤草、阿胶;气虚型加黄芪、白术、扁豆、薏苡仁),1剂/d,水煎300m L,早中晚饭后30min服用;西药治疗同对照组。连续治疗28d为1疗程。观测临床症状、不良反应。连续治疗2疗程(56d),判定疗效。[结果]治疗组痊愈117例,显效39例,无效12例,总有效率92.86%。对照组痊愈69例,显效32例,无效49例,总有效率67.33%。治疗组疗效优于对照组(P<0.01)。[结论]凉血五根汤联合西药治疗血热妄行型过敏性紫癜疗效满意,无严重不良反应,值得推广。

[Objective]To observe the Liangxue Wugen soup combined with western medicine blood heat Wang Xing allergic purpura effect.[Methods]Randomized parallel controlled method,318 cases of outpatient and inpatient law by simple dice randomly divided into two groups.Prevention and control of infection,use of antibiotics or antiviral drugs when clear upper respiratory infection or cold;pay attention to rest,to prevent fatigue,avoid taking drugs and may cause allergic foods such as onion,garlic,pepper,wine,seafood.150 cases in the control group ebastine,10mg/time,1 time/d;cetirizine drops,1m L/second,1/d;or other upper respiratory infection with cefprozil tablet,0.5g/times 1 times/d;TWP,1mg/kg,3 times/d;vitamin C,0.1g/ second,1/d;rutin,20mg/time,3 times/d;severe allergic reactions to the compound glycyrrhizin,50mg/time,3 times/d;severe abdominal pain atropine,0.3mg/time,3 times/d;joint pain to excellent Bufen,50mg/time,3 times/d;vomiting blood in the stool to Western microphone nizatidine,0.2g/times,2 times/d.Treated 168 cases of Liangxue Wugen Soup（Baimaogen 30 g,Zicaogen 15 g,Qiancaogen 20 g,Shengdi 30 g,Diyu 20 g,Huaihua 15g;articulated plus Qinjiu,Duhuo,Mugua;abdominal plus Baishao,Xuanhu,Gancao;renovascular increase Daji,Xiaoji,Xianhe cao,A＇jiao;Deficiency Huangqi,Baizhu,Biandou,Yiyiren）;a/d,decoction 300 m L,breakfast,dinner after taking 30min;Western medicine with the control group.28 d for a course of continuous treatment.Observation of clinical symptoms,adverse reactions.Continuous treatment of 2 courses（56d）,to determine efficacy.[Results]The cure 117 cases,effective in 39 cases,12 cases,the total efficiency of 92.86%.The control group cured 69 cases,effective in 32 cases,49 cases ineffective,the total efficiency of 67.33%.Treatment group than the control group（P〈0.01）.[Conclusion]Liangxue Wugen Soup combined with western medicine cooling blood heat Wang Xing allergic purpura results were satisfactory,no side effects,worthy of promotion.
