辨证分型治疗中风随机平行对照研究

Syndromes Stroke Randomized Parallel Group Study Treatment

[目的]观察辨证分型治疗中风疗效。[方法]使用随机平行对照方法,将80例住院患者按抛硬币法简单随机分为两组。对照组40例吡拉西坦,0.8g/次,3次/d;华佗再造丸,6g/d,3次/d。治疗组40例出血性中风:250m L 0.9%氯化钠+40m L清开灵,1次/d,静滴;脑水肿予脱水降颅内压治疗;感染用抗生素;肝阳暴亢-天麻钩藤饮(天麻10g,石决明20g,钩藤、怀牛膝、杜仲、桑寄生各15g;痰多加川贝、天竺黄各10g),1剂/d,水煎200m L,早晚口服。风瘀阻络-秦艽汤(白芍、茯苓、生地黄、秦艽各15g,白术、白僵蚕、胆南星、羌活各10g,全蝎4.5g)。缺血性中风:肝阳暴亢-20m L丹参注射液+500m L5%葡萄糖,1次/d,静滴;滋阴祛痰汤(石决明、生牡蛎、代赭石各15g,钩藤嫩、龟板炙、滁菊花各12g,明天麻、胆南星各9g,怀牛膝,白芍各15g,川贝粉3g),1剂/d,水煎200m L,早晚口服。风瘀阻络-半夏白术天麻汤(秦艽、当归各10g,白术15g,茯苓12g,白茯苓15g,熟地黄、甘草各10g,法半夏、天麻各9g,陈皮10g,兼表征祛风散邪,加白芷、防风、细辛;肝风重,适当增减钩藤),1剂/d,水煎200m L,早晚口服。连续治疗90d为1疗程。观测临床症状、不良反应。治疗1疗程,判定疗效。[结果]治疗组痊愈8例,显效16例,有效13例,无效3例,总有效率92.50%。对照组痊愈4例,显效3例,有效13例,无效10例,总有效率75.00%。治疗组疗效优于对照组(P<0.05)。[结论]辨证分型治疗中风疗效满意,无严重不良反应,值得推广。

[Objective]To observe the effect of the treatment of stroke syndrome type.[Methods] Randomized,parallel controlled method,80 patients were hospitalized method is simple coin toss randomized into two groups.The control group of 40 patients Piracetam,O.Sg/time,3 times/d;Huatuozaizao pill,6g/d,3 times/ d.Treatment group,40 cases of hemorrhagic stroke：250mL 0.9% sodium chloride＋4OmL Qingkailing, 1 times/ d,intravenous infusion;dehydration of brain edema intraeranial pressure lowering treatment;infections with antibiotics;liver-yang hyperactivity-TGD（Tianma 10g,Shijueming 20g,Gouteng,Huainiuxi,Duzhong,Sangjisheng 15g;phlegm Duojia chuanbei,Tianzhuhuang each lOg）,l/d,decoction 200mL,sooner or later orally.Wind stasis- Gentiana soup（Baishao,Fuling,Shengdihaung,Qinjiu 15g,Baizhu,Baijiangcan,Dannanxing,Qianghuo lOg,Quanxie 4.Sg）.Ischemic stroke：liver yang hyperactivity-20mL Salvia Injection＋500mL 5% glucose,intravenous infusion, l/d;Yin expectorant soup（Shijueming,Shengmuli,Daizheshi 15g,Gouteng,Guiban,Juhua each 12g,Mingtianma,Dannanxing 9g, Huainiuxi,Baishao 15g,Chaunbeifen 3g）,l/d,decoction 200mL,sooner or later orally.Wind stasis-Banxiabaizhutianma soup（Qinjiu,Danggui 10g,Baizhu 15g,Fuling 12g,Baifulingl 5g,Shu dihuang,Gancao 10g,Fabanxia,Tianma each 9g,Chenpi 10g,and characterization of expelling loose evil,plus Baizhi,Fangfeng,Xinxin;liver wind heavy,appropriate changes Gouteng）,a/d,decoction 200mL,sooner or later orally.90d for a course of continuous treatment.Observation of clinical symptoms,adverse reactions.One course of treatment,to determine efficacy.[Results]The cure eight cases,16 cases markedly effective in 13 cases,3 cases,the total efficiency of 92.50%.The control group recovered four cases,effective in 3 cases,effective in 13 cases,10 cases,the total efficiency of 75.00%.Treatment group than the control group（P〈0.05）.[Conclusion]Differential Diagnosis and Treatment Satisfaction stroke effect,no side effects,worthy of promotion.
