静脉注射氨茶碱治疗硬脊膜穿刺后头痛的临床研究 被引量：3

摘要 硬脊膜穿刺后头痛（Post-dural puncture headache,PDPH）是腰椎穿刺术最常见的并发症,临床表现为前额、枕部疼痛或弥漫性头痛,疼痛的性质为钝痛或波动性痛,立位时加重,平卧位减轻.流行病学资料显示,约10％～30％的患者在腰椎穿刺术后48 h内会出现PDPH[1～3],在肥胖的年轻女性和妊娠女性中PDPH的发病率会更高[4].PDPH常常伴有颈强直、耳鸣、听力减退、畏光及恶心等症状,多数患者头痛会在站立或行走时加重,严重的影响了患者的日常生活,给患者带来了极大痛苦[5],因此探索PDPH的有效治疗方法显得尤为重要.近年来国外曾有小样本报道静脉或口服茶碱可有效治疗PDPH[6],国内尚无此类报道.我们在国际上首次使用静脉注射氨茶碱代替茶碱治疗PDPH患者1 1例,取得了显著疗效,且无明显不良反应,报道如下.
