鲁西南地区小儿包皮发育状况调查

Investigation of prepuce development situation among children in southwest of shandong Province

目的：通过调查男童阴茎包皮发育状况，找出包皮发育规律，为包皮异常的诊治原则提供理论依据。方法以鲁西南（枣庄、济宁、菏泽）0~18岁男性作为调查对象，按年龄分为5组：0~3岁为第1组，4~6岁为第2组，7~10岁为第3组，11~14岁为第4组，15~18岁为第5组。手法上翻包皮，然后记录包皮可上翻的程度，并依此分为Ⅰ-Ⅶ度。结果各年龄组包茎率依次为80.54%，74.63%，56.41%，22.31%，20.21%，随着年龄的增长，包茎、包皮过长、包皮粘连的发生率均逐渐下降，呈负相关（r =-0.98，P =0.004）。7~18岁经常外翻清洗包皮组与不常清洗组的 I - VI 度包皮数在统计学上有显著性差异；7~10岁Ⅶ度包皮比例开始明显增加。结论包皮过长、包茎在儿童中普遍存在，大多数儿童的包皮在青春期能逐渐完全上翻，所以对包皮过长者建议在11岁以后手术治疗。

Objective：This survey aims to find out prepuce development laws and to provide scientific evidence for diag-nosis and treatment of prepuce by investigating prepuce development situation of male children. Method：Select infants, primary school and middle school students in the Southwest of Shandong Province（Zaozhuang,Jining and Heze）as the re-search objects,divide them into five groups according to their ages：children of 0 - 3 years is Group 1,3 ~ 6 years is Group 2,7 ~ 10 years is group 3,11 ~ 14 years is group 4,15 ~ 18 years is group 5. Upturn the prepuce with hand,then keep a record of the upturning degrees and classify these records. Results：Phimosis rates of each group respectively are 80. 54% , 74. 63% ,56. 31% ,33. 33% ,10. 98% . Phimosis rate drops with the increase of age;the pertinence is r = - 0. 98,P =0. 004. Prevalence rate of phimosis,redundant prepuce and the foreskin adhesion gradually drops with the increase of age. There is a significant difference statistically between groups of 7 ~ 18 age with I - VI phimosis rate who often accept prepuce eversion cleaning and who don’t often accept prepuce eversion cleaning. Ⅶ phimosis rate of 7 ~ 10 age group is markedly increased. Conclusion：Redundant prepuce and phimosis are ubiquitous among children;most can be upturned in their pu-berty. For those who need treatment for phimosis,it is advised that surgical treatment can be conducted after they are 11 years old.
