我院门急诊超说明书用药处方分析

Analysis on Off-Label Drug Use at Outpatient and Emergency Department in Our Hospital

目的：对我院门急诊超说明书用药情况进行统计分析。方法：采取回顾性方法,对我院2013年~2014年实施处方点评的门急诊处方中存在超说明书用药的不合理处方进行统计分析。结果：在所点评的2400张处方中,超说明书用药处方共86张,主要包括超剂量（39.53%）、超用药频次（30.23%）、超适应症（16.28%）和超用药途径（13.95%）。结论：我院门急诊超说明书用药现象较多,存在医疗安全隐患。建议医院建立相应的管理制度与流程,规范医师用药行为,以规范医师用药行为,保证患者的用药安全。

Objective： This article aims to investigate the situation of off-label drug use at outpatient and emergency department in our hospital for references of rational drug use in the clinic. Method： With the retrospective method, the irrational prescriptions in respect of off-label drug use in outpatient and emergency department from 2013 to 2014 were selected and analyzed. Results： Of the total 2400 prescriptions, 86 ones involved off-label drug use. The problems were mainly over-dosage （39.53%）, over frequency of drug use （30.23%）, beyond indication （l 6.28%） and beyond the route of medication （13.95%）. Conclusions： The off-label drug use is widespread at outpatient and emergency department in our hospital. Risks exist during the health care process. It is sensible for our hospital to establish corresponding control system and procedure, so as to ensure the doctors＇ rights and interests, and to ensure safe use of drug in patients.
