CMV-DNA与CMV-IgM对婴幼儿巨细胞感染的诊断价值

摘要 目的:研究尿液CMV-DNA联合血清CMV-Ig M检测对婴儿巨细胞感染的诊断价值。方法:选取2012年8月到2014年8月期间我院疑似巨细胞感染的婴幼儿70例,应用化学发光法检测血清CMV-Ig M,采用实时荧光定量PCR法(FQ-PCR)来检测尿液CMV-DNA,并比较经抗病毒治疗前和治疗后检测结果的变化。结果:单独检测尿CMV-DNA的阳性率为62.8%(44/70),单独检测血清CMV-Ig M的阳性率为55.7%(39/70),两者比较差异无统计学意义(P>0.05);两者联合检测的阳性率为77.1%(54/70),与两种方法单独检测比较差异具有统计学意义(P<0.05);经抗病毒治疗以后两种检测结果均显著降低,与治疗前比较差异具有统计学意义(P<0.05)。结论:尿液CMV-DNA联合血清CMV-Ig M检测能显著提高婴儿巨细胞感染的阳性率。 Objective: To study the diagnostic value of CMV-DNA in urine and serum CMV-Ig M detection to cytomegalovirus infection in infants. Methods: 70 cases suspected CMV infection in newborns were selected from our hospital during the period from August 2012 to August 2014, CMV-Ig M in serum were detected in chemiluminescence, CMV-DNA in urine were detected in real-time quantitative PCR(FQ-PCR), before antiretroviral therapy and after treatment changed were compared. Results: Individually tested positive rate of CMV-DNA in the urine was 62.8%(44/70), separate tested positive rate of CMV-Ig M in serum was 55.7%(39/70), the difference was no statistically significant(P 〉0.05); the positive rate of combined detection was 77.1%(54/70), the difference was statistically significant compared with individually tested with both methods(P〈0.05); two detection results were significantly lower after treatment by the anti-virus, the difference was statistically significant compared with before treatment(P〈0.05). Conclusion: CMV-DNA in urine and serum CMV-Ig M detection can significantly improve the positive rate of cytomegalovirus infection in infants.
