64排螺旋CT诊断变应性支气管肺曲霉菌病的价值

Value of 64-slice spiral CT in the diagnosis of allergic bronchopulmonary aspergillosis

目的分析变应性支气管肺曲霉菌病(ABPA)的螺旋CT表现,提高对其诊断准确率。方法回顾性分析我院10例经临床确诊ABPA患者的螺旋CT影像资料,进行总结分析。结果中心性支气管扩张10例,扩张的支气管腔内粘液栓形成8例,其中5例粘液栓呈高密度,浸润性实变6例,树芽样影像表现4例。结论 ABPA的螺旋CT表现的主要特征为中心性支气管扩张,且在扩张的支气管管腔内可见粘液栓,部分为高密度粘液栓,此特征性CT表现结合患者临床症状及实验室检查结果基本可以确诊ABPA。

Objective To analyze the spiral CT features of allergic bronchopulmonary aspergillosis （ ABPA） in order to improve the accuracy rate of diagnosis. Methods The spiral CT features of 10 patients with ABPA were retrospectively analyzed. Results The CT showed 10 cases with central bronchiectasis and 8 cases with mucoid im-paction of dilated bronchial, including 5 cases with high density of mucus plugs, 6 cases with patchy shadows, and 4 cases with tree buds signs. Conclusion The relative characteristics of ABPA include central bronchiectasis, and bronchial mucus can be seen in the intracavitary of dilated bronchi, some of which is high-density mucus plug.Therefore, this feature makes ABPA easily diagnose while combined with the CT features, clinical and laboratory ex-amination.
