中药注射剂超说明书用药评价与药物利用研究分析

Evaluation of Off-label Use of TCM Injection and Analysis of Drug Utilization

目的:评价分析中药注射剂的超说明书用药情况,促进中药注射剂的合理应用。方法:随机抽取2014年出院病例共计360份,通过与中药注射剂药品说明书的一致性分析,评价中药注射剂超说明书用药情况,并对用药金额、用药频度、限定日费用、排序比等药物利用数据进行研究,综合评价医院2014年中药注射剂临床使用情况。结果:360份病历中有135份使用了中药注射剂,使用率为37.5%,其中有58例存在不同类型的超说明书用药情况,占42.96%。各类中药注射剂均存在超说明书用药现象,以注射用血塞通为代表的理血剂使用频率相对较高,选择广泛。结论:中药注射剂临床应用广泛,超说明书用药发生率较高,应加强中药注射剂临床应用管理,促进其使用的合理性和安全性。

Objective： To evaluate and analyze the situation of the off-label use of TCM injections, so as to promote the rational use of TCM injection. Methods： A total of 360 patients discharged in 2014 were randomly selected for the study. Consistency between clinical use and dispensatory of TCM injection was evaluated. The utilization of TCM injection was statistically analyzed in respects of consumption sum, DDDs, DDC and sorting ratio. In addition,the clinical application of TCM injection in 2014 was evaluated. Results： Of 360 medical records, 135 cases received TCM injections, with a utilization rate of 37.5%. There were 58 cases of different types of off-label drug use, accounting for 42.96%. Off-label drug use widely existed in all kinds of TCM injections. The blood-regulating recipes such as Xuesaitong for injection were relatively in a high off-label drug use. Conclusion： The clinical application of TCM injections is widespread, and the off-label use rate is high. It is necessary to strengthen the management of clinical application of TCM injections, and to promote the rational use and safety.
