不明原因肝衰竭在肝衰竭的病因构成中占有较高的比例。回顾了不明原因肝衰竭的流行病学、病因学、治疗及预后的研究进展。认为隐匿性HBV感染、疱疹病毒感染、输血传播或庚型肝炎感染、人细小病毒B19感染、自身免疫性肝炎等是不明原因肝衰竭发生的可能病因。对不明原因肝衰竭患者可考虑采用阿昔洛韦进行经验性治疗。不明原因肝衰竭患者病死率高,危害大,应当加强相关病因及治疗手段的研究。

A high proportion of the causes of liver failure remain unknown. This paper reviews the progress in the epidemiology, etiology, treatment, and prognosis of liver failure of unknown etiology. The possible causes of liver failure of unknown etiology may include occult hepatitis B virus infection, herpesvirus infection, transfusion - transmitted virus infection, hepatitis G virus infection, human parvovirus B19 infection, autoimmune and hepatitis. Aciclovir can be considered in the empirical treatment for patients with liver failure of unknown etiology. The mortality in patients with liver failure of unknown etiology is high. The research on the etiology and treatments should be strengthened.
