进展期白癜风患者血清免疫球蛋白和补体、抗黑素细胞抗体及TGF-β水平的检测

The Expression of Serum Immunoglobulin,Compliment,Anti-melanocyte Antibody and Transforming Growth Factor-β in Active Vitiligo

目的研究进展期白癜风患者血清中免疫球蛋白和补体、抗黑素细胞抗体及转化生长因子-β(TGF-β)水平及其临床意义。方法收集35例进展期白癜风患者血清,免疫散射比浊法检测其血清免疫球蛋白及补体的水平,采用酶联免疫吸附试验(ELISA)方法检测其血清中抗黑素细胞抗体和TGF-β水平,并与20例健康人对照。另外,进展期白癜风患者均予综合治疗,疗程结束后评定疗效。结果进展期白癜风患者血清Ig G和Ig A及C3水平均显著低于正常人对照组,血清抗黑素细胞抗体水平明显高于正常人对照组,差异均有统计学意义(P均<0.05);血清Ig M,C4和TGF-β水平与正常人对照组的差异均无统计学意义(P均>0.05)。进展期白癜风患者的血清抗黑素细胞抗体水平比较,治疗有效组显著高于治疗无效组,且差异有统计学意义(P<0.05);其血清TGF-β,lg G,Ig A,Ig M,C3和C4水平比较,治疗有效组与治疗无效组的差异均无统计学意义(P均>0.05)。结论进展期白癜风患者存在全身免疫异常,血清中抗黑素细胞抗体异常高表达可能与其临床疗效相关。

Objective To investigate the levels and their clinical significance of serum immunoglobulin, compliment and anti - melanocyte antibody and transforming growth factor - β （ TGF - β） in active vitiligo. Methods Thirty - five patients with active vitiligo were included in this study as the treatment group. Immune turbidimetry was used to detect the levels of immunoglobulin and compliment. Enzyme - linked immunosorbent assay （ELISA） was used to detect the serum levels of anti - melanocyte antibody and TGF - [3. Results were com- pared with controls of 20 healthy people. All patients received comprehensive treatment. Clinical efficacy was evaluated at the end of the study. Results The levels of lgG, IgA and C3 in serum were significantly lower in patients than in normal controls （ all P 〈 0.05 ）. The levels of anti - melanocyte antibody in serum of patients were significantly higher than that in normal controls （ P 〈 0.01 ）. There were no significant differences in levels of IgM, C4, and TGF -β between patients and normal controls （ all P 〉 0.05）. Among these 35 patients, the level of anti - melanocyte antibody was significantly higher in effective patients than in ineffective patients （ P 〈 0. 05 ）, while there were no significant differences in levels of TGF - β, lgG, IgA, IgM, C3 , and C4 between effective patients and ineffective patients （ all P 〉 0. 05 ）. Conclusion Systemic immune abnormalities existed in active vitiligo patients. The high expression of serum anti - melanocyte anti- body might be related to the clinical efficacy.
