表现为皮肤感染的原发性皮肤鳞状细胞癌1例

A Case of Primary Cutaneous Squamous Cell Carcinoma Mimicking Skin Infection

患者女,56岁。右前臂伸侧皮疹伴红肿、疼痛10d。皮疹位于右前臂伸侧中部,初为红斑基础上的脓疱疹,逐渐增大伴脓液、中央结痂,抗生素治疗无效。5d后,右前臂近肘部再发一枚类似皮疹。两枚皮疹均有压痛,皮温升高。皮损活检后经组织病理检查诊断为:高分化鳞癌,予手术切除。本病例提示,对于难以治愈的皮肤感染,需考虑皮肤肿瘤的可能性。

A 56-year-old female presented with painful red rash on her right forearm for 10 days. The rash had begun as an erythematous papulopustule and gradually enlarged with exudation and crust. Antibiotic treatment was in- effective and a similar lesion appeared on her elbow after 5 days. Both lesions had tenderness and raised skin temperature. Based on the histopathology result, the final diagnosis was well - differentiated squamous cell carcinoma. This case illustrates an awareness to consider possible malignancy when face with refractory skin infections.
