摘要
目的:比较脐动脉多普勒阻力指数增加的胎儿椎动脉和大脑中动脉的脑保护机制情况。方法:回顾性分析1 354例胎儿的椎动脉(vertebral artery,VA)、大脑中动脉(middle cerebral artery,MCA)和脐动脉(umbilical artery,UA)的多普勒检查结果。结果:胎儿UA多普勒频谱显示舒张期血流时,VA和MCA的阻力系数(Resistance Index,RI)没有差异,当UA缺乏舒张期血流,VA和MCA的RI比较低。结论:当UA的血流严重受影响时,UA和MCA同时降低阻力,启动大脑保护机制,保证大脑的氧供应。
Objective: To compare the status of brain protection mechanism of fetal vertebral artery( VA) and middle cerebral artery( MCA)with Doppler umbilical arterial resistance index( RI) increased. Methods: The results of Doppler examination of VA,MCA and umbilical artery( UA) in 1354 fetuses were analyzed retrospectively. Results: When fetal UA Doppler spectrum showed diastolic blood flow,there was no difference in the resistance index between VA and MCA. When there was the lack of UA diastolic blood flow,RI of VA and MCA was relatively low. Conclusion: When the blood flow of UA was severely affected,UA and MCA simultaneously lowered resistance,initiated brain protection mechanism and ensured oxygen supply of brain.
