摘要
目的探讨T2DM合并周围动脉疾病(PAD)患者血浆网膜素-1水平与炎症因子及相关代谢指标的关系。方法 114例T2DM患者分为T2DM+PAD组和单纯T2DM组,各57例。另选正常对照者(NGT)68名。采用ELISA测定各组血浆网膜素-1水平、炎症指标及代谢参数,并对各变量行相关分析、多元逐步回归分析及Logistic回归分析。结果血浆网膜素-1水平在NGT组、T2DM、T2DM+PAD组逐渐降低(P<0.01)。血浆网膜素-1水平与TNF-α、IL-6、高敏C反应蛋白(hsC-RP)、FPG、HbA1c、TC呈负相关(r=-0.338、-0.225、-0.219、-0.263、-0.342、-0.235、-0.334,P<0.05或P<0.01),与踝肱指数(ABI)及HDL-C呈正相关(r=0.444、0.358,P<0.01)。多元逐步回归分析显示,TNF-α、HbA1c和TC是血浆网膜素-1独立相关因素;Logistic回归分析结果显示,血浆网膜素-1、TNF-α、IL-6及HbA1c是T2DM患者发生PAD的影响因素。结论 T2DM合并PAD与网膜素-1降低相关,网膜素-1表达可能会受到炎症因子的影响。
Objective To investigate the relationship between plasma omentin-1 level and inflammatory cytokines in T2 DM patients with PAD. Methods A total of 114T2 DM patients and 68 healthy subjects were selected and divided into three groups:T2DM with PAD group(T2DM +PAD group,n=57),T2 DM without PAD group(T2DM group,n=57)and normal control group(NGT group,n=68).Plasma omentin-1level,inflammatory cytokines and metabolic parameters were measured in three groups,which were analyzed by correlation analysis,multiple stepwise regression analysis and logistic regression analysis. Results Plasma omentin-1levels of NGT group,T2 DM group and T2DM+PAD group decreased in sequence(P〈0.01).Plasma omentin-1level was negatively correlated with TNF-α,IL-6,hsC-RP,FPG,HbA1 c and TC(r=-0.338,-0.225,-0.219,-0.263,-0.342,-0.235,-0.334,P〈0.05 or P〈0.01)and was positively correlated with ABI and HDL-C(r=0.444,0.358,P〈0.01).Multiple stepwise regression analysis showed that TNF-α、HbA1c and TC were independent related factors for plasma omentin-1.Logistic regression analysis showed that plasma omentin-1,TNF-α、IL-6and HbA1 c were influencing factors for T2 DM in combination with PAD. Conclusion Decrease of serum omentin-1level is associated with PAD in T2 DM patients.Expression of omentin-1 may be influenced by inflammatory cytokines.
