摘要
目的观察IGR患者血清血红素氧化酶-1(HO-1)、颈动脉内-中膜厚度(CIMT)及斑块形成情况,探讨HO-1在IGR伴或不伴动脉粥样硬化(AS)患者中的变化及影响因素。方法选取IGR者199例,接受颈动脉超声检测,根据病变性质分为颈动脉正常(CIMT≤0.9mm)组124例和颈动脉斑块形成(CIMT≥1.5mm,或有斑块形成)组75例,另设对照(NGT)组62名。ELISA检测血清HO-1水平。结果颈动脉正常组和颈动脉斑块形成组HO-1水平高于NGT组,且颈动脉斑块形成组高于颈动脉正常组(P〈0.05);Spearman相关分析显示,HO-1水平与SUA、TC、LDL-C、BMI呈正相关(r=0.126、0.204、0.187、0.207,P=0.042、0.001、0.003、0.002);多元逐步回归分析显示,HOMA-IR、TC、CIMT是HO-1的危险因素(β=50.05、26.16、572.68,P=0.000、0.007、0.001);Logistic回归分析发现,HO-1为颈动脉粥样硬化的保护因素(OR=0.990,95%CI:0.981~0.999,P=0.000)。结论 IGR患者检测血清HO-1水平可能有益于发现高AS患病风险者。
Objective To investigate the change of HO-1level and its influencing factors in IGR patients with/without atherosclerosis. Methods A total of 199 IGR patients and 62 healthy subjects were envolved in this study.IGR patients were divided into two groups according to CIMT detected by ultrasound:IGR patients with normal carotid artery group(CIMT≤0.9mm,n=124),IGR patients with carotid plaque group(CIMT≥1.5 mm or have plaque,n=75).Serum level of HO-1 was tested by ELISA. Results Serum level of HO-1was higher in IGR group than in NGT group and also higher in IGR patients with carotid plaque group than in IGR patients with normal carotid artery group(P〈0.05).Spearman correlation analysis showed that serum HO-1level was positively correlated with SUA,TC,LDL-C and BMI(r=0.126,0.204,0.187,0.207,P=0.042,0.001,0.003,0.002).Multiple stepwise regression analysis showed that HOMA-IR,TC and CIMT were risk factors for serum HO-1level(β=50.05,26.16,572.68,P=0.000,0.007,0.001).Logistic regression analysis showed that serum HO-1was a protective factor for carotid atherosclerosis(OR=0.990,95%CI:0.981~0.999,P=0.000).Conclusion Serum HO-1level test may be useful for identifying IGR patients with high risk of atherosclerosis.
