眼外伤致继发性青光眼的临床研究

Clinical study of secondary glaucoma caused by ocular contusion

目的探讨眼外伤继发青光眼的原因与治疗效果。方法回顾性分析86例(共计86只眼)眼外伤继发青光眼患者的临床资料和治疗方法。结果眼外伤致继发性青光眼的主要原因包括眼内出血型42只眼(48.9%)、房角挫伤型21只眼(24.4%)、晶状体源性15只眼(17.4%)、炎症粘连型8只眼(9.3%)。根据致病原因制定不同的治疗方案,79只眼眼压被控制,成功率为91.9%;72只眼(83.7%)的视力与术前相同或提高;14只眼(16.3%)视力下降。结论眼外伤致继发性青光眼机制复杂,治疗应根据个体差异选择合理的治疗方法。

Objective To study the causes and treatment of secondary glaucoma caused by ocular contusion. Methods The data of 86 cases（ 86 eyes） of secondary glaucoma caused by ocular contusion were retrospectively reviewed. Results Mainly reaons of secondary glaucoma caused by ocular contusion included intraocular bleeding in 42 eyes（ 48. 9%）,angle recession in21eyes（ 24. 4%）,lens dislocation in 15 eyes（ 17. 4%）,and proliferation type in 8 eyes（ 9. 3%）. Different treatment options were developed on basis of different causes. The intraocular pressure（ IOP） was successfully controlled in 79 eyes（ 91. 9%）;sights of 72 eyes（ 83. 7%） were steady or enhanced in comparison to pre surgery; sights of 14 eyes（ 16. 3%） became worse.Conclusion The secondary glaucoma caused by ocular contusion are complex which should be treated by different methods according to cause and complication.
