摘要
目的:探讨微螺钉种植体支抗在成人双颌前突正畸中应用效果。方法:115例行矫治的成人双颌前突患者据矫治方法不同分为种植体支抗组(n=59)和传统支抗组(n=56),种植体支抗组采取微螺钉种植体支抗,传统支抗组采取传统方式支抗,所有患者均拔除4颗第一前磨牙,利用MBT金属托槽矫治器进行矫治。记录两组患者主动矫治的时间,利用X线头颅侧位片测量对矫治前后效果进行评价,记录两组患者矫治过程中并发症发生情况。结果:种植体支抗组患者矫治时间为(19.9±3.2)m,显著短于传统支抗组的(21.3±3.7)m,差异有统计学意义(P<0.05)。矫治后,两组患者SNA角较矫治前无差异(P>0.05),两组患者上中切牙角和下中切牙角均较矫治前减少,而上下中切牙长轴交角则较矫治前增加,差异均有统计学意义(P<0.05);种植体支抗组患者矫治后上中切牙角和下中切牙角均小于传统支抗组,而上下中切牙长轴交角则大于传统支抗组,差异均有统计学意义(P<0.05);两组患者均顺利完成矫治,未出现局部感染、肿胀等并发症,未发生矫治器松动和脱落等。结论:微螺钉种植体支抗应用于成人双颌前突正畸可有效缩短患者主动矫治时间,达到前牙内收、双颌前突矫治的目的。
Objective: To investigate the micro-screw implant anchorage in adult patients with bimaxillary protrusion orthodontic application effect. Methods: 115 routine treatment of adult patients with bimaxillary protrusion divided according to the different treatment methods for implant group (n=59) and traditional anchorage group (n=56), set to take micro-implant anchorage screw implant anchorage anticonventional anchorage anchorage group take the traditional approach, all patients were plucking four first premolars, the use of MBT metal bracket appliance for correction. After the treatment, no difference (P〉0.05) compared two groups of patients before treatment SNA angle, the two groups of pa- tients before and lower incisor angle corner cisor reduction compared with treatment, while the upper and lower central incisors are more long axis angle correction before the increase, the differences were statistically significant (/9〈0.05); after treatment on the patient group for implant angle and lower incisor incisor angle less than traditional anchorage group, while the upper and lower incisor long axis angle greater than conventional anchorage group, the difference was statistically significant (P〈0.05); patients were successfully completed treatment, local infection, swelling and other complications does not appear, did not occur appliance loose and fall off. Orthodontics can effectively shorten the front sudden conclusion: applies micro-screw implant anchorage active treatment of adult patients with bimaxillary time, closing to within the front teeth, bimaxillary protrusion treatment purposes.
