CT virtual endoscopy in diagnosis of tracheobronchial foreign bodies in one cases of fishbone

1病例报告患者,男,67岁,因＂咳嗽伴间断咯血5d,胸痛呼吸困难2d＂于2012年6月16日急诊入院。患者5d前进食鱼汤呛咳后出现咳嗽、少量痰中带血,咳出少量鱼刺后自觉好转,之后间断咯血。当地诊所给予青霉素治疗3d,咳嗽较前好转,无咯血。2d前再次出现痰中带血,伴右侧胸痛及刺激性干咳,胸痛为持续性钝痛,夜间较重,伴呼吸困难。

Summary Patients, male, 67 years old, had been coughing with intermittent hemoptysis for 5 days, chest pain and dyspnea for 2 days was hospitalized. Physical examination： T 36.6C , P 80 beats/min, R 21 beats/min, BP 155/90 mmHg, conscious, smooth breathing, lips without cyanosis, no rale in the lung, no abnormal signs were found in heart and abdomen. Blood gas analysis= pH 7. 426, PO2 70.9 mmHg, PCO2 32.1 mmHg. Bio- chemical examination, blood coagulation function is normal. ECG is normal. Chest CT： foreign body in the right main bronchial, CT simulation imaging： long strip foreign body can be seen in the right main bronchial, bronchos copy： a long strip foreign body was visible in the right main bronchial, with surface coated by white secretions. The foreign body was a fishbone.
