ASIR技术在膀胱肿瘤模型低剂量CT扫描中的应用研究 被引量:1

摘要 目的:通过对膀胱肿瘤模型行多排螺旋CT不同剂量下扫描,对获得图像行自适应统计迭代重建(Adaptive statistical iterative reconstruction。ASIR)及CT仿真膀胱镜成像(CTVC),比较各种不同剂量条件下所得图像的质量,分析ASIR技术在膀胱肿瘤低剂量CT检查中的应用价值。方法:选用一新鲜正常猪膀胱,在其黏膜表面通过结扎法获得20枚1-10mm(≤5mm13枚,〉5mm7枚)连续的模拟肿瘤,运用高分辨CT(GE optima CT660)在不同剂量(120kVp,20、40、60、80、100、120、140、160、180、200mA)下扫描,并分别应用11种不同ASIR水平(0、10%、20%、30%、40%、50%、60%、70%、80%、90%及100%)进行数据重建,获得CTVC图像,对不同采集条件、不同ASIR水平的图像进行客观和主观评价,测量并比较平均噪声、平均信噪比(SNR)、平均对比噪声比(CNR)及CTVC图像的肿瘤检出率。结果:在管电流一定情况下,各组图像经80%水平ASIR重建后,其噪声呈较明显下降(P〈0.05)。在相同管电流下,随着ASIR水平的增高,图像的SNR值逐渐升高(P〈0.05)。在相同管电流下,随着ASIR水平的增高,图像的CNR值逐渐升高(P〈0.05)。CTVC图像对膀胱肿瘤的检出率在管电流一定的情况下,各病灶的检出率随ASIR水平的升高并无明显提高(P〉0.05)。结论:ASIR技术可以提高膀胱低剂量CT图像的质量,降低患者所接受的辐射剂量,但是,ASIR技术提高膀胱CTVC图像病灶检出率的能力有限,需要更多的实验进行深入研究。 Objective: To compare the quality and lesion conspicuity of images of bladder tumor model acquired on CT by standard-dose and low-dose with adaptive statistical iterative reconstruction(ASIR) and CT virtual eystoscopy(CTVC) imag- ing, and to evaluate the application of ASIR technique for low dose CT scanning of bladder tumors. Methods: A fresh porcine bladder was adopted and ligated 20 lumps (1-10 nam, ≤5 mm 13 lumps, 〉5 mm 7 lumps) to simulate bladder tumors. High resolution CT(GE optima CT660) with different doses(120 kVp, 20, 40, 60, 80, 100, 120, 140, 160, 180, 200 mA) scan- ning was used, and 11 different levels of ASIR (0, 10%, 20%, 30%, 40%, 50%, 60%, 70%, 80%, 90% and 100%) for data reconstruction were performed to get CTVC images. After objective and subjective evaluation of images collected in different conditions of different ASIR levels, the average noise, signal-to-noise ratio(SNR), contrast to noise ratio(CNR) and tumor detec- tion rate were compared and analyzed. Results: Under the same tube current, the noise of images decreased obviously with ASIR level of 80%(P〈0.05). Under the same tube current, image SNR increased gradually with the increase of ASIR levels(P〈 0.05). Under the same tube current, image CNR increased gradually with the increase of ASIR levels (P〈0.05). Under a con- stant tube current, the lesion detection rate did not increase obviously with the increase of ASIR levels (P〉0.05). Conclusion: ASIR technique can improve the quality of low-dose CT images of the bladder with reduction of radiation dose. ASIR tech- nique cannot obviously improve the lesion detection rate of the bladder CTVC images.
