常规放射治疗传统技术联网数字化的研究应用可行性分析

摘要 目的 常规放射治疗传统技术联网数字化的研究应用可行性分析。方法 选择2010年1月~2013年12月我院接诊的32例鼻咽癌患者进行研究,按照入院的时间顺序分为观察组和对照组。全部病例按规范化要求制定治疗计划。采用低熔点铅挡块、面颈联合野等中心照射技术。鼻咽根治剂量68~70 Gy/7周,颈淋巴结阳性者颈部根治剂量60~70 Gy/6~7周,阴性者给予预防剂量50 Gy/5周。观察组采用常规放射治疗传统技术联网数字化治疗技术,均根据CT和MRI对照射靶区进行个体化设计;而对照组采用传统常规放射治疗技术。在治疗后1个月,根据治疗前后头颈部CT肿瘤的变化情况进行客观近期疗效判定,统计治疗后1~3年的生存率。结果 治疗后,观察组患者的总有效率93.75%高于对照组的62.5%,两组比较其差异具有统计学意义(P〈0.05);观察组的鼻咽癌患者完全缓解(CR)5例,部分缓解(PR)10例,无变化(NC)1例,与对照组相比,差异具有统计学意义(P〈0.05);两组患者比较治疗后的1年、2年、3年生存率,观察组的生存率分别为93.75%、81.25%、68.75%,而对照组为62.50%、43.75%、31.25%,两组比较其差异具有统计学意义(P〈0.05)。结论 采用常规放射治疗传统技术联网数字化技术,CT和MRI进行照射靶区的个体化设计,改进照射精度,提高照射剂量,加强放疗全过程的质量控制与质量保证,有助于提高鼻咽癌常规放疗的疗效。常规放射治疗传统技术联网数字化技术还有减少差错率、减少人力物力投入,节约成本、投资,省时省力等优点,值得广泛推广。 Objective To study the feasibility of com- bination of conventional radiation therapy with traditional technology and digital network. Methods Thirty-two pa- tients with Nasopharyngeal Carcinoma from January 2010 to December 2013 in our hospital were included. Each patient was given a treatment plan according to standardized requirements. Low melting point lead block, face, neck and other joint center field irradiation technique was used. The radical dose for nasopharynx is 68~70 Gy/7 weeks, the radical neck dose for cervical lymph node-positive is 60~70 Gy/6~7 weeks, and for the negative lymph node was a prophylactic dose of 50 Gy/5 weeks. Observation group using conventional radio- therapy treatment of traditional technology and digital network technology, which is based on CT and MRI to design the individualized irradiation target; while the control group using traditional conventional radiotherapy techniques. Therapeutic efficacywas evaluated according to the changes of tumor on head and neck CT 1 month after treatment, and the survival rate of one year, two-year, three-year after treatment. Results After treatment, the total effective rate of the observation group(93.75%)were higher than the control group(62.5%), the difference between the two groups was statistically significant (P〈 0.05) ; In the observation group, 5 cases got complete remission (CR) , 10 cases had partial remission (PR) , only 1 case showed no change (NC) . Compared with the control group, the difference was statistically significant (P 〈 0.05) ; The survival rates of observation group of 1-year, 2 years, 3 years were 93.75%, 81.25%, 68.75%, while that of the control group was 62.50%, 43.75%, 31.25% respectively. And the difference between the two groups was statistically significant (P〈 0.05) . Conclusion The use of combination of conventional radiation therapy and digital network technology can significantly enhance the efficacy, improve the accuracy of radiotherapy, reduce errors, reduce the human and material resources, save cost, investment, time and effort, etc., and be worthy of promotion in clinic.
