糖尿病性皮肤病与糖尿病慢性并发症的相关性分析

Correlation study of diabetic dermopathy with chronic complications in type 2 diabetes mellitus

目的：探讨糖尿病性皮肤病（DD）与糖尿病慢性并发症之间的相关性。方法收集我院2型糖尿病（T2DM）患者210例，其中合并 DD 者100例（DD 组），无 DD 者110例（NDD 组）。记录并比较两组患者的糖尿病病程、糖尿病性肾病（DN）、糖尿病性视网膜病变（DR）、糖尿病性周围神经病变（DPN）、糖尿病性自主神经病变（DAN）、下肢动脉粥样硬化（LEAS）、非酒精性脂肪肝（NAFLD）以及高血压、糖尿病足、冠心病、脑梗塞病史，以有无 DD 为因变量进行 Logistic 回归分析。结果DD 组患者中，DN、DR、DPN、DAN、LEAS 患者的比例明显高于 NDD 组（两组分别为73．00％与24．55％，76．00％与17．27％，48．00％与27．27％，26．00％与13．64％，54．00％与38．18％）；DD 组糖尿病病程明显长于 NDD 组[（10．94±3．92）年与（6．90±2．98）年，P ＜0．05]。二元 Logistic 回归分析结果显示，糖尿病病程、DN、DR、DPN 和 DD 的发生有独立相关性（P ＜0．05或 P ＜0．01）。结论在2型糖尿病患者中，糖尿病病程、DN、DR、DPN 与 DD 独立相关，且对于2型糖尿病患者合并 DD 时应注意糖尿病慢性并发症的筛查。

Objective To explore the correlation of diabetic dermopathy（DD）with chronic com-plications in type 2 diabetes mellitus（T2DM）.Methods Through the same period of hospitalization,210 patients with T2DMwere studied,which included 100 patients with DD（DD group）and 110 cases with no diabetic dermopathy（NDD group）.The records of duration of diabetes,diabetic nephropathy（DN）,diabet-ic retinopathy（DR）,diabetic peripheral neuropathy（DPN）,diabetic autonomic neuropathy（DAN）,lower extremity atherosclerosis（LEAS）,nonalcoholic fatty liver disease（NAFLD）and history of hypertension, diabetic foot,coronary artery disease and cerebral infarction between the two groups were analyzed.Logis-tic regression was applied in patients with DD and NDD group.All the measurement data was expressed as （mean ±standard deviation）and was analyzed by t-test.The numerical data was analyzed by chi-square test.Results The ratio of DN,DR,DPN,DAN,LEAS in DD group were higher than those in NDD group [73.00% vs.24.55%,76.00% vs.17.27%,48.00% vs.27.27%,26.00% vs.13.64%,54.00% vs. 38.18%]（P 〈0.05 or P 〈0.01）.The level of duration of diabetes in DD group was higher than those in NDD group[（10.94 ±3.92）years vs.（6.90 ±2.98）years]（P 〈0.01）.The results of binary logistic re-gression analysis showed that diabetes duration,DN,DR,DPN were still independently related to DD in T2DM（P 〈0.05 or P 〈0.01）.Conclusion Duration of diabetes,DN,DR,DPN are independently cor-related with DD in T2DM.Diabetic chronic complications should be screened in type 2 diabetic patients with DD.
