我院肺移植中心建立以个案管理护士为基础的多专业合作综合管理模式,在肺移植患者接受治疗的不同阶段,根据个案管理护士的职责实施"多对一"服务模式,为患者和家属提供了贯穿整个治疗过程的专业化、个体化的全程服务,取得了较好效果。

The lung transplantation center in our hospital established a nurse-based case management model with a multi-discipline cooperation and comprehensive management. In the different stages of treatment in lung transplantation, case management nurses delivered ＂multi to one＂ service model for patients with lung transplantation and their families according to their unique responsibility, and provided professional, individualized full-range service during the whole treatment. The model achieved good effects.
