心力衰竭患者血清CA125及BNP水平变化研究

Research on the Changes of Serum CA125 and BNP Levels in Patients with Chronic Heart Failure

目的:探讨慢性心力衰竭(CHF)患者血清CA125、BNP水平变化与病情严重程度的相关性。方法:于2013年4月-2014年4月随机选取经病史、体格检查、超声心动图等证实为CHF的患者200例,并根据NYHA标准,分为心功能Ⅱ级组74例、Ⅲ级组53例、Ⅳ级组73例,再随机选取心功能正常者40例作为对照组,比较各组别之间的CA125、BNP、射血分数(LVEF)水平以及相关性。结果:4组患者的血清CA125、血浆BNP、LVEF比较,差异具有统计学意义(P<0.05),Ⅱ级组、Ⅲ级组、Ⅳ级组的血清CA125和BNP明显高于对照组(P<0.05),并且随着级别越高,CA125与BNP的含量越多,LVEF越低;CA125和BNP的含量与LVEF是呈负相关性(P<0.05)。结论:CHF患者的血清CA125、血浆BNP与NYHA的心功能分级有着明显的相关性,CA125和BNP可以作为CHF的诊断以及预后追踪的实验室指标。

Objective To explore the relationship between severity of heart function impairment and changes of serum CA125 and BNP levels in patients with chronic heart failure. Methods From April 2013 to April 2014,the data of 200 patients with chronic heart failure comfirmed by medical history,physical examination,echocardiography were collected and divided into heart function grade Ⅱ group（ 74 cases）,grade Ⅲ group（ 53 cases） and grade Ⅳ group（ 73 cases） according to the standard of NYHA,and then 40 patients without heart failure were randomly selected as control group. The levels of CA125,BNP and left ventribuler ejeciton fraction（ LVEF） were comparatively analyzed and their correlation was analyzed. Results The levels of CA125,BNP and LVEF during four groups were significant differences（ P〈0. 05）. The serum levels of CA125 and BNP of grade Ⅱ,Ⅲ,Ⅳ groups were obviously higher than that of control group（ P〈0. 05）,and the levels of CA125 and BNP were increased following the grade increasing,but LVEF decreased. The levels of CA125 and BNP were negatively correlated with LVEF（ P〈0. 05）. Conclusion There is a significant correlation between the levels of CA125 and BNP and the classification of NYHA,and the levels of CA125 and BNP could be uesd as valuable markers to diagnose CHF and predict therapeutic effect.
