乳腺肿瘤边缘剪切波速度诊断乳腺肿瘤性质价值 被引量:1

Value of marginal shear wave velocity to the diagnosis of breast benign and malignant tumors

摘要 目的探讨应用声触诊组织成像量化技术检测乳腺肿瘤边缘剪切波速度诊断乳腺肿瘤良、恶性质的准确性。方法经手术组织病理证实乳腺恶性肿瘤患者80例和良性肿瘤患者160例,均于术前应用声触诊组织成像量化技术检测肿瘤内部、边缘及正常腺体组织剪切波速度;绘制ROC曲线计算肿瘤内部及边缘剪切波速度诊断乳腺良、恶性肿瘤的灵敏度、特异度。结果良性、恶性肿瘤内部剪切波速度[(2.3±0.7)、(7.7±2.6)m/s]及肿瘤边缘剪切波速度[(2.2±0.9)、(6.6±3.1)m/s]均高于正常腺体组织[(1.5±0.6)、(1.6±0.7)m/s](P<0.05),恶性肿瘤内部剪切波速度高于边缘(P<0.05),恶性肿瘤内部及边缘剪切波速度值均明显高于良性肿瘤(P<0.05);肿瘤边缘剪切波速度值诊断乳腺恶性肿瘤的AUC为0.87,诊断界值为3.5 m/s;肿瘤内部剪切波速度值诊断乳腺恶性肿瘤的AUC为0.93,诊断界值为3.9m/s;与组织病理结果进行对照,肿瘤边缘剪切波速度≥3.5m/s诊断乳腺恶性肿瘤的灵敏度为71.25%,特异度为98.13%;肿瘤内部剪切波速度≥3.9m/s诊断乳腺恶性肿瘤的灵敏度为88.75%,特异度为95.63%。结论乳腺肿瘤边缘剪切波速度诊断乳腺肿瘤良、恶性质具有较好的准确性。 Objective To investigate the diagnostic accuracy of virtual touch tissue quantification technique (VTQ) in the detection of marginal shear wave velocity (SWV) for the diagnosis of breast benign and malignant tumors. Methods Eighty patients with malignant breast tumors and 160 with benign breast tumors confirmed by postoperative histopathological examination were detected the SWV in the tumor center, tumor margin and normal breast gland tissue by VTQ. The ROC curve was drawn to calculate the diagnostic accuracy and specificity of marginal and internal SWV for breast benign and malignant tumors. Results The internal SWV values ((2.3±0.7), (7.7±2.6) m/s) and the marginal SWV values ((2.2±0.9), (6.6±3.1) m/s) of the benign and malignant tumors were significantly higher than those in normal breast gland tissue ((1.5±0.6), (1.6±0.7) m/s) (P〈0.05), the internal SWV was higher than marginal SWV in malignant tumors (P〈0.05), and the internal and the marginal SWV values were higher in malignant tumors than those in benign tumors (P〈0.05). The AUC of the marginal SWV for malignant tumors was 0.87, and the cut-off value was 3.5 m/s. The AUC of the internal SWV for malignant tumors was 0.93, and the cut-off value was 3.9 m/s. Compared with the pathological results, the sensitivity of marginal SWV ≥3.5 m/s was 71.25 0/00 and the specificity was 98.13% for malignant tumors, and the sensitivity of internal SWV ≥3. 9 m/s was 88. 75% and the specificity was 95. 63% for malignant tumors. Conclusion Marginal SWV has a high accuracy for diagnosing benign and malignant breast tumors.
