超声内镜引导下经食管针吸活检术对肺癌诊断价值的探讨 被引量:2

摘要 目的探讨超声内镜引导下经食管针吸活检术(EUS-TENA)对肺癌诊断和分期的价值。方法临床疑诊肺癌患者12例,胸部CT示肺部病变临近食管并伴有纵隔淋巴结肿大,使用富士上消化道超声电子内镜系统和COOK 22G超声活检针,经食管对病变实施针吸活检。结果 12例患者均获得足够的标本量,病理诊断:6例为小细胞癌,4例为腺癌,2例为鳞癌,均未发生与活检相关的并发症。结论 EUS-TENA术是一种安全、简便、实用的检查方法,患者耐受性好,是对超声支气管镜引导下经支气管针吸活检术(EBUS-TBNA)和纵隔镜检查的有效补充,对肺癌的诊断和分期具有较高的临床应用价值。 【Objective】To evaluate the clinic value of transesophgeal endoscopic ultrasound-guided needle aspiration(EUS-TENA) for diagnosing and staging lung cancer.【Methods】Twelve patients with suspected lung cancer,of whom chest CT showed positive paraesophageal lung lesions with mediastinal lymph nodes, were submitted to upper gastric endoscopic system(FUJI) and 22 G Cook Echo Tip biopsy needle aspiration through the esophagus.【Results】Adequate quantity of samples were procured from these 12 patients, and processed appropriately. A definitive pathological diagnosis was obtained. Six patients had small cell carcinoma, four patients had adenocarcinoma, and two patients had squamous cell carcinoma. None of the patients had complications associated with needle biopsy as-piration. 【Conclusion】EUS-TENA is a safe, convenient and effective diagnostic method with high patient compliance, which can complement endobronchial ultrasound guided transbronchial needle aspiration(EBUS-TBNA) and mediastinoscopy effectively in term of diagnostic reach. EUS-TENA has a high clinic value in diagnosing and staging lung cancer.
