个案管理模式护理对脑卒中后抑郁患者主要照顾者的影响

Effect of case management on primary caregivers for patients with poststroke depression

目的 探讨个案管理护理对脑卒中后抑郁患者主要照顾者的影响。方法 选择2012年6月-2014年10月佛山市第一人民医院特需医疗中心96例脑卒中后抑郁患者的主要照顾者为研究对象,按患者的入院时间顺序将其主要照顾者分为对照组（52名）和实验组（44名）。对照组接受常规护理,实验组实施个案管理护理。两组均接受出院后6个月随访。分别收集入院时、出院时及出院后3、6个月两组主要照顾者的自我效能感（GSES）和生活满意度（LSIA）,比较两组护理效果。结果 入院时、出院时两组GSES、LSIA比较,差异无统计学意义（P〉0.05）。出院后3、6个月的GSES、LSIA实验组均优于对照组,差异均有统计学意义（P〈0.05或P〈0.01）。结论 个案管理护理可以有效提高脑卒中后抑郁患者主要照顾者的自我效能感及生活满意度。

Objective To discuss the effect of case management on primary caregivers for patients of post-stroke depression. Methods A total of 96 primary caregivers of patients with PSD were recruited in the Special Medical Center of the First People′s Hospital of Foshan City In Guangdong Province from June 2012 to October 2014, and they were divided into control group（52 cases） and study group（44 cases） according to admission time of patients. The control group was given routine care while the study group was given case management, and both received a 6- month followup visit. Then collected general self-efficacy scale（GSES） and life satisfactory index A（LSIA） of all primary caregivers when admission, discharge, the third month and sixth month after discharge respectively were collected, and the nursing effect in two groups were compared. Results When admission and discharge, the GSES and LSIA of the two group had no significant difference（P〈0.05）, but 3 month and 6 month after discharge, the comparative results of study group were all superior to the control group with significant difference（P〈0.05 or P 〈0.01）. Conclusion Case management can improve self-efficacy and life satisfaction of the primary caretakers of patients with post-stroke depression.
