脑白质疏松症发生机制研究进展 被引量:6

摘要 作为脑小血管病(small veseel disease,SVD)的重要组成部分,脑白质疏松症(leukoaraiosis,LA),也称脑白质损害(white matter lesions,WMLs)或脑白质高信号(white matter hyperintensities,WMHs),是指由多种原因引起的脑室周围或皮质下脑白质的斑点状或斑片状改变,在CT上显示低密度,在MRI-T1加权像上显示等信号或低信号,T2加权像和液体衰减反转恢复序列(fluid-attenuated inversion recovery images,FLAIR)上呈现高信号改变[1].目前认为,LA是引起血管性认知功能障碍(vascular cognitive impairment,VCI)主要的影像学改变之一[2],然而其发生机制尚不清楚,也是当前研究的热点.本文将从慢性缺血/低灌注学说、血脑屏障破坏学说、内皮功能障碍学说、炎症反应学说、遗传学说、静脉异常学说等几个方面对LA发生的神经机制进行阐述,以期对临床诊治提供一定的理论依据.
