摘要
目的:研究老老年原发性高血压病(EH)患者中医证素分布特点,为临床中医辨证论治提供更多的依据。方法:选择2013年7月-2014年7月中国中医科学院广安门医院心内科住院的年龄≥80岁的EH病患者357例;对纳入患者进行高血压中医症状调查问卷,录入Epidata软件,将所导出的数据运用因子分析法进行分析,得出不同证素的频数分布情况。结果:病位证素:肝肾同病(32.77%)>脾肾同病(31.10%)>心(9.52%)>心肝肾同病(8.68%)>肾(7.56%)>肝(6.16%)>心肝同病(4.21%);病性证素:痰浊+气滞+血瘀+阳亢(22.69%)>阳虚+血瘀(16.25%)>水停+痰浊(14.85%)>阴虚+痰浊+湿困(10.08%)>阴虚+气虚(8.68%)>血瘀+气虚(9.52%)>痰浊+血瘀(7.56%)>气滞+血瘀(6.16%)>痰浊+血瘀+气滞(4.21%)。结论:老老年EH证素分布中病位证素以肝肾同病为主;病性证素为多证素组合,主要为虚实夹杂证素;故建议老老年EH的中医诊治应虚实兼顾。
Objective:To study the old elderly patients with primary hypertension(essential,hypertension,EH) in patients with TCM syndrome distribution characteristics,may provide more evidence for clinical syndrome differentiation in traditional Chinese medicine.Methods:2013 July-2014 July years of age or older hospitalized in the department of cardiology,Guang'anmen Hospital at the age of 80,358 patients with essential is selecting;TCM symptoms questionnaire for patients enrolled in,input Epidata software,using factor analysis method,carries on the analysis,obtains the different syndrome distribution of pigment frequency condition.Results:The syndrome elements of disease location:liver and kidney disease(32.77%) spleen and kidney disease(31.10%) heart(9.52%) liver and kidney disease(8.68%) kidney(7.56%) liver(6.16%) heart disease(4.21%);nature of disease syndrome:qi stagnation+ blood stasis+ phlegm +yang kang(22.69%) yang deficiency+ blood stasis(16.25%) water stop+phlegm(14.85%) yin + phlegm + wet trapped(10.08%) qi and yin deficiency(8.68%) blood stasis and qi deficiency(9.52%) phlegm + blood stasis(7.56%) qi stagnation + blood stasis(6.16%) phlegm + blood stasis + qi Stagnation(4.21%).Conclusion:Liver and kidney is the main disease location of very elderly hypertensive patients;the nature of disease syndrome is complicated with real and deficiency,diagnosis and treatment of them should be taking into account the actual situation.
