高频超声对20例手足部腱鞘肿块的诊断分析

Analysis for high-frequency ultrasound in diagnosing thecal cyst of amphi-tendon

目的探讨高频超声对腱鞘肿块的诊断价值。方法回顾性分析经手术病理证实20例手足部腱鞘肿块的声像图表现,并与病理结果进行对照分析。结果 20例手足部的腱鞘肿块中囊性、实质性分别有12例、8例,病理证实腱鞘囊肿12例、腱鞘巨细胞瘤1例、腱鞘纤维组织细胞瘤3例、腱鞘纤维瘤4例。高频超声对6例腱鞘囊肿作出诊断正确,4例误诊为滑膜囊肿,1例误诊为滑膜炎,1例误诊为实性低回声肿块;高频超声对1例腱鞘巨细胞瘤、3例纤维组织细胞瘤及3例纤维瘤均作出实性病变,1腱鞘纤维瘤误诊为腱鞘囊肿。结论高频超声能较好鉴别腱鞘肿块的囊、实性,对腱鞘囊肿诊断准确性较高。

Objective To analyze the value of high-frequency ultrasound in diagnosing thecal cyst of amphi-tendon. Methods The sonograms of 20 patients with thecal cyst of amphi-tendon on extremity confirmed by surgery and pathology were analyzed retrospectively by comparing with pathologic result. Results 12 cases were of thecal cyst; 1 case of giant cell tumor of tendon sheath; 3 cases of fibrohistiocytoma of tendon sheath; 4cases of fibroma of tendon sheath. 6 cases thecal cyst were misdiagnosed as synovial cyst,4 cases as synonitis,1case as parenchymatous tumeur by high-frequency ultrasound. 1 case of giant cell tumor of tendon sheath,3cases of fibrohistiocytoma of tendon sheath and 3 cases of fibroma of tendon sheath were diagnosed as parenchymatous tumeur. 1 cases of fibroma of tendon sheath was misdiagnosed as thecal cyst. Conclusions Highfrequency ultrasound plays an important role in diagnosis and can provide comparatively accurate diagnosis of thecal cyst.
