摘要
目的探讨一期原位修薄的游离股前外侧穿支皮瓣修复手部创面的可行性。方法应用一期原位修薄的游离股前外侧穿支皮瓣移植修复手部创面15例。皮瓣大小为4 cm×7 cm^10 cm×16 cm。结果手术时间为4~5 h。15例皮瓣完全成活。1例皮瓣远端边缘有2 cm×3 cm坏死,经换药后愈合。15例均获得随访,时间6~12个月。皮瓣外观及质地良好,外形不臃肿。结论一期修薄的游离股前外侧穿支皮瓣能满足手部创面修复的特殊要求,血供可靠,外形良好,无需二期修薄。
Objective To explore the feasibility of the in situ one-stage thinning of the anterolateral thigh perforator flap for repairing the hand defects. Methods 15 cases with hand defects were treated with anterolateral thigh perfo-rator flap. The flap size ranged from 4 cm × 7 cm to 10 cm × 16 cm. Results The operating time was 4~5 h. 15 cases survived completely. Only one flap had a size of 2 cm × 3 cm necrosis and the complication was solved after Af-ter dressing. At a 6~12 months follow-up, the flaps took well with good contours. The thickness of these flaps was half of those harvested with common ways. Conclusions In situ one-stage thinning of the anterolateral thigh perfora-tor flap is a good technique for the hand defects. It has the advantages of the reliable blood supply, more satisfactory contours and requires no secondary thinning.
