维生素D结合蛋白基因rs2282679 A/C多态性与慢性阻塞性肺疾病的相关性研究 被引量:6

摘要 目的探讨维生素D结合蛋白(VDBP)基因rs2282679 A/C多态性与慢性阻塞性肺疾病(COPD)的相关性。方法选取2012年3月—2014年5月衡水市哈励逊国际和平医院住院治疗的男性COPD患者120例为COPD组,另选取同时期衡水市疾病预防控制中心体检健康男性112例为对照组,采用酶联免疫吸附试验(ELISA)法检测血清25羟维生素D〔25(OH)D〕水平,采用Taq Man-聚合酶链反应检测VDBP基因rs2282679 A/C等位基因频率及基因型分布。结果对照组25(OH)D水平(55.1±5.8)nmol/L高于COPD组的(37.3±2.3)nmol/L(t=31.220,P<0.001)。对照组与COPD组VDBP基因rs2282679 A/C等位基因频率与基因型分布比较,差异均有统计学意义(P<0.05)。对照组与COPD组AA、AC、CC基因型者组间25(OH)D水平比较,差异均有统计学意义(P<0.05)。对照组和COPD组组内AA、AC、CC基因型者25(OH)D水平比较,差异均有统计学意义(P<0.05);其中AC、CC基因型者低于AA基因型者,CC基因型者低于AC基因型者(P<0.05)。调整年龄、吸烟指数后,AA基因型比AC基因型COPD易患率低18.3%〔OR=0.374,95%CI(0.197,0.708),P=0.002〕;AA基因型比CC基因型COPD易患率低35.8%〔OR=0.260,95%CI(0.106,0.640),P=0.002〕;AC基因型比CC基因型COPD易患率低16.7%〔OR=0.201,95%CI(0.849,4.744),P=0.109〕。结论 VDBP基因rs2282679 A/C多态性与COPD具有相关性,其中CC基因型与COPD易患性、25(OH)D水平关系紧密,或可作为COPD易患性筛选的突变位点。 Objective To investigate the correlation between rs2282679 A/C polymorphism in vitamin D- binding protein( VDBP) gene and chronic obstructive pulmonary disease( COPD). Methods Enrolled 120 COPD patients who were admitted into Harrison International Peace Hospital in Hengshui from March 2012 to May 2014 as COPD group. Also enrolled 112 health males who received physical examination in the same period as control group. ELISA was employed to detect serum25( OH) D,and Taq Man- PCR was used to detect the allele frequency and genotype distribution of rs2282679 A / C in VDBP gene. Results The 25( OH) D levels of control group and COPD group were( 55. 1 ± 5. 8) nmol / L and( 37. 3 ± 2. 3)nmol / L respectively,with control group higher than COPD group( t = 31. 220,P〈0. 001). Control group and COPD group were significantly different in allele frequency and genotype distribution of rs2282679 A / C in VDBP gene( P〈0. 05). Control group and COPD group were significantly different in 25( OH) D levels of AA,AC and CC genotypes( P〈0. 05). In control group and COPD group,subjects of AA,AC and CC genotypes were significantly different in 25( OH) D level( P〈0. 05);subjects of AC and CC genotype were lower than subjects of AA genotype,and subjects of CC genotype were lower than subjects of AC genotype( P〈0. 05). After the adjustment of age and smoking indexes,AA genotype is 18. 3% lower than AC genotype in susceptibility to COPD 〔OR = 0. 374,95% CI( 0. 197,0. 708), P = 0. 002 〕; AA genotype is 35. 8% lower than CC genotype in susceptibility to COPD 〔OR = 0. 260,95% CI( 0. 106,0. 640),P = 0. 002〕; AC genotype is 16. 7% lower than CC genotype in susceptibility to COPD 〔OR = 0. 201,95% CI( 0. 849,4. 744),P = 0. 109 〕. Conclusion rs2282679 A / C polymorphism in VDBP gene is significantly associated with COPD. CC genotype is related to the liability of COPD and is closely related with 25( OH) D,and it may be a mutation site for the screening of COPD liability.
