摘要
目的探讨2型糖尿病患者C反应蛋白和瘦素表达水平与胰岛素抵抗的相关性。方法选取2010年3月~2012年7月在我院内分泌科就诊的2型糖尿病患者166例作为观察组,并按照肥胖程度分为T2DM组(80例)和T2DM合并肥胖组(86例),同时选取同期体检健康者45例为对照组。检测空腹血糖(FPG)、空腹胰岛素(FINs)、C反应蛋白(CRP)、瘦素(1eptin)并计算胰岛素抵抗指数,利用Pearson相关性分析探讨糖尿病患者胰岛素抵抗的相关原因。结果观察组FPG、FINs、CRP、Leptin、HOMA-IR均显著高于对照组(t=8.642、10.720、5.961、6.325、5.467,P〈0.05)。合并T2DM肥胖组CRP、Leptin、HOMA-IR分别为(9.28±1.96)mg/L,(13.95±2.18)μg/L,(7.02±0.43),显著高于T2DM组(t=4.923、4.780、4.584,P〈0.05)。Pearson相关性分析显示,糖尿病患者的胰岛素抵抗与C反应蛋白(CRP)以及瘦索(Leptin)间呈正相关(r=0.462、0.352,P〈0.05)。结论血浆CRP、lepfin的表达量升高时可引起胰岛素抵抗.从而成为引发2型糖尿病的危险因素。
Objective To explore the correlation among the C-reactive protein, leptin levels and insulin resistance of patients with type 2 diabetes. Methods 166 cases with type 2 diabetic were taken as observation group which were grouped into T2DM group(n=80) and T2DM with obesity group(n=86) according to the degree of obesity from March 2010 to July 2012. Meanwhile, 45 healthy cases were selected in the same period as the control group. The correlation between the expression of C-reactive protein, leptin levels and insulin resistance of patients with type 2 diabetes were investigated by determining the fasting blood glucose(FPG), fasting insulin (FINs), C-reactive protein (CRP), leptin, and insulin resistance index. Results The FPG, FINs, CRP, leqtin, and HOMA-IR in observation group were significantly higher than those in control group (t=8.642, 10.720, 5.961, 6.325, 5.467, P〈0.05). The CRP, leptin, and HOMA-IR of T2DM with obesity group were (9.28±1.96) mg/L, (13.95±2.18) μg/L, (7.02±0.43) respectively, which were significantly higher than those of the T2DM group(t=4.923, 4.780, 4.584, P〈0.05). Pearson correlation analysis indicated that C re- active protein (CRP) and leptin were positively correlated with the insulin resistance of diabetic patients (r=0.462, 0.352, P〈0.05). Conclusion The expression of plasma CRP, leptin can cause the increase of insulin resistance, and become the risk factor of type 2 diabetes.
