Cyberwand双导管碎石清石术与气压弹道碎石术治疗复杂性肾结石的效果比较

Effect comparison of Cyberwand dual probe lithotripter and pneumatic ballistic lithotripsy in the treatment of complex renal calculus

目的比较Cyberwand双导管碎石清石术与气压弹道碎石术治疗复杂性肾结石的临床效果和安全性。方法选取本院2011年1月~2014年1月收治的200例复杂性肾结石患者作为研究对象,随机分为观察组和对照组,各100例。观察组采用Cyberwand双导管碎石清石术治疗,对照组采用气压弹道碎石术治疗,比较两组的手术时间、术中出血量、术后住院时间、术后并发症等指标。在治疗后7、15、30 d分别进行超声检查泌尿系统残余结石情况。结果两组的术中出血量、术后住院时间及术后并发症发生率比较,差异无统计学意义（P〉0.05）。观察组的手术时间显著短于对照组,差异有统计学意义（P〈0.05）。观察组治疗后7、15、30 d的残余结石数量显著少于对照组,差异有统计学意义（P〈0.05）。观察组的总有效率为98.0%,显著高于对照组的82.0%,差异有统计学意义（P〈0.05）。结论 Cyberwand双导管碎石清石术治疗复杂性肾结石具有结石清除率高、手术时间短、出血少、并发症少等优点,是治疗体积较大、多发性、鹿角性及铸型等复杂肾结石的理想方法。

Objective To compare the clinical efficacy and safety of Cyberwand dual lithotripter and pneumatic ballistic lithotripsy in the treatment of complex renal calculus. Methods 200 patients with definite diagnosis of complex renal calculi From January 2011 to January 2014 in our hospital were selected and randomly divided into the observation group and the control group,100 cases in each group.The observation group was treated with Cyberwand dual probe lithotripter,the control group was treated with pneumatic ballistic lithotripsy.The operation time,intraoperative amount of bleeding,hospital stay after surgery and postoperative complication in two groups was compared.The residual stone was reexamined by ultrasound for urinary system 7,15,30 days after surgery. Results There was no statistical difference in intraoperative amount of bleeding,hospital stay after surgery,or postoperative complications in both groups（P0.05）.The operation time in the observation group was shorter than that in the control group,with significant difference（P0.05）.The number of residual stones in the observation group after treatmeng for 7,15,30 days was less than that in the control group,with significant difference（P 0.05）.The total effective rate in the observation group was 98.0%,which was higher than 82.0% in the control group,with significant difference（P0.05）. Conclusion In the treatment of complex renal calculus,Cyberwand dual lithotripter has advantages of high stone clearance,short operation time,little amount of bleeding and few complication,which is an ideal method to treat complex renal calculus with the characteristics of large size,multiple occurrence,staghorn,and cast.
