带缝匠肌骨瓣移植联合空心钉内固定治疗青少年股骨颈骨折的临床效果分析

Clinical effect analysis of sartorial-pedicled ilium bone transplantation combined with hollow screw fixation treating femoral neck fracture of youngsters

目的分析带缝匠肌骨瓣移植联合空心钉内固定对青少年股骨颈骨折的治疗效果。方法选取本院2012年7月~2014年6月收治的80例青少年股骨颈骨折患者,按随机数字表法分成研究组和对照组,各40例。研究组采用带缝匠肌骨瓣移植术联合空心钉内固定术进行治疗;对照组单纯采用空心螺钉内固定术进行治疗。比较两组手术时间、术中出血量、住院时间、股骨头缺血性坏死发生率、骨折愈合时间、临床疗效及治疗前、后生活质量。结果研究组手术时间长于对照组（P〈0.05）;研究组术中出血量较对照组多（P〈0.05）;研究组住院时间及骨折愈合时间均较对照组短（P〉0.05）;研究组股骨头缺血性坏死发生率低于对照组（P〈0.05）;研究组临床疗效优良率高于对照组（P〈0.05）;两组治疗前生活质量得分比较差异无统计学意义（P〉0.05）;研究组治疗后生活质量得分高于对照组（P〈0.05）。结论带缝匠肌骨瓣移植术联合空心钉内固定治疗青少年股骨颈骨恢复快,综合疗效好。

Objective To analyze the effect of sartorial-pedicled ilium bone transplantation combined with hollow screw fixation treating femoral neck fracture of youngsters. Methods 80 youngsters with hollow screw fixation in our hospital from July 2012 to June 2014 were selected and divided into research group and control group randomly,each group for 40 cases.The research group was treated with sartorial-pedicled ilium bone transplantation combined with hollow screw fixation.And the control group was only treated with hollow screw fixation.The operation time,bleeding amount in operation,length of stay,incidence rate of ischemic necrosis of femoral head,fracture healing time,clinical efficacy and life quality before and after treatment between two groups were compared. Results The operation time of research group was longer than that of control group（P0.05）.The bleeding amount in operation of research group was more than that of control group（P0.05）.The length of stay and fracture healing time of research group was shorter than that of control group respectively（P0.05）.The incidence rate of ischemic necrosis of femoral head of research group was lower than that of control group（P 0.05）.The excellent rate of the efficacy of research group was higher than that of control group（P0.05）.There was no evident difference for the life quality before treatment between two groups（P0.05）.The life quality score of research group after treatment was higher than that of control group（P 0.05）. Conclusion Sartorial-pedicled ilium bone transplantation combined with hollow screw fixation treating femoral neck fracture of youngsters has the advantages of rapid recovery and good efficacy.
