多层螺旋计算机断层扫描薄层增强三期扫描对小肝癌患者的诊断作用

Diagnostic value of small hepatocellular carcinoma with triple-phase multi-slice helical computed tomography

摘要 目的分析多层螺旋计算机断层扫描(CT)薄层增强三期扫描对小肝癌患者的诊断作用。方法回顾性分析秦皇岛市第四医院2013年6月至2014年5月间收治的83例经病理证实的小肝癌患者的临床资料,总结多层螺旋CT薄层增强三期(动脉期、门脉期和延迟期)增强扫描结果。结果 83例患者中经病理证实病灶89个,病灶直径0.8~2.9 cm,单个癌结节77例,2个癌结节6例;动脉期、门脉期和延迟期的检出率分别为82.0%(73/89)、80.9%(72/89)和86.5%(77/89),但差异无统计学意义(P〉0.05);而三期综合检查率最高,为97.8%(87/89),与3期检出率进行两两比较,差异均有统计学意义(均P〈0.05)。结论多层螺旋CT薄层增强三期扫描对小肝癌患者的诊断作用肯定,延迟期检出率相对较高,综合多层螺旋CT薄层增强三期扫描可显著提高诊断能力。 Objective To evaluate the diagnostic value of small hepatocellular carcinoma with triple-phase multi-slice helical computed tomography( CT). Methods From June 2013 to May 2014 in the Fourth Hospital of Qinhuangdao,the clinical data of 83 small hepatocellular carcinoma patients confirmed by pathology were retrospectively analyzed,and the enhanced scan results of triple-phase multi-slice helical CT( arterial,portal and late phases) were summed. Results The pathological examinations demonstrated that 89 lesions in 83 cases,and the diameter of the lesion was 0. 8 ~ 2. 9 cm,77 cases with single nodules,6 cases with two nodules; the detection rate of arterial,portal and late phases were 82. 0%( 73 /89),80. 9%( 72 /89),86. 5%( 77 /89) respectively. The detection rate of late phases was slightly higher than those of arterial and portal late phases,but the difference was not statistically significant( P〉0. 05),and three comprehensive examination rate was higher [97. 8%( 87 /89) ],compared to three pair wise detection rate,the differences were statistically significant( P〈0. 05). Conclusions Triple-phase multi-slice helical CT for small hepatocellular carcinoma has definite diagnosis effect,and the detection rate of late phases is relatively high. Complex triple-phase multi-slice helical CT imaging can significantly improve diagnostic capabilities.
