6550例妇女宫颈人乳头状瘤病毒感染状况分析

Analysis of the infection status of female cervical HPV in 6550 cases

目的分析天台县妇女宫颈人乳头状瘤病毒（HPV）感染情况及亚型分布。方法采用HPV PCR反向斑点杂交技术对6 550例宫颈脱落细胞标本,进行23种HPV基因分型检测。结果 6 550例样本中,检出HPV阳性者1 928例,总阳性率为29.44%,检出22种亚型,未检出基因型HPV 82。HPV基因型单一感染1 319例（68.41%）,多重感染609例（31.59%）。不同年龄组中HPV感染高峰在≤20岁、51岁~和〉60岁3个年龄段,感染率分别为67.11%、34.37%和32.68%。天台县女性宫颈HPV感染主要基因型别为HPV 16型、HPV 58型、HPV 52型、HPV 6型、HPV 43型、HPV33型,依次占总感染型别的14.45%、11.67%、11.28%、6.53%、6.07%、5.64%。高危型感染数占69.01%,可见高危型感染问题的严重性。结论对≤20岁及〉50岁的妇女应予以重视,对阳性患者要加强定期随访,对育龄妇女进行HPV基因分型检测有利于宫颈癌的早期预警和治疗。

Objective To investigate the human papillomavirus（ HPV） infection status and the genotypes distribution of female cervical in Tiantai. Methods The samples of cervical exfoliated cells were detected by HPV PCR reverse dot blot hybridization genotyping for 23 kinds of HPV genotypes. Results In 6 550 cases,1 928 cases were HPV positive,the total positive rate was29. 44%,22 kinds of genotypes were detected,genotype HPV 82 was not detected. 1 319 cases were the infection of single HPV genotype（ 68. 41%）,609 cases were the multiple infection of HPV genotypes（ 31. 59%）. The peaks of HPV infection were in age groups of ≤20,51- 60 and 〉60,and the infection rates were respectively 67. 11%,34. 37% and 32. 68%. The main infection subtypes of female cervical in Tiantai were HPV 16,HPV 58,HPV 52,HPV 6,HPV 43 and HPV 33,and the infection rates were respectively 14. 45%,11. 67%,11. 28%,6. 53%,6. 07% and 5. 64%. Type of high- risk cases accounted for 69. 01% of total cases of infection,which was a serious problem. Conclusion The age groups of ≤20 and 50 years should be pay more attention,to the positive patients,we should strengthen the regular follow- up. The detection of HPV genotypes and subtypes is in favor of early warning and early treatment of cervical cancer.
