自发性荨麻疹血清总IgE、补体与Th1/Th2相关细胞因子的检测 被引量:13

Detection and Significance of Serum Total Ig E, Complements and Correlation of Th1/Th2 Cytokines in Spontaneous Urticaria Patients

摘要 目的探讨血清总Ig E、补体与Th1/Th2相关细胞因子在自发性荨麻疹(SU)发病中的意义,并分析各检测指标水平与病情程度的相关性。方法双抗体夹心酶联免疫吸附实验(ELISA)法与双光径免疫浊度分析仪(IMMAGE)检测80例急性自发性荨麻疹(ASU)、170例慢性自发性荨麻疹(CSU)患者和38例健康对照者血清总Ig E、IL-2、IL-4、IFN-γ、C3、C4水平,并将其表达水平与病情程度和评分进行相关性分析。结果 CSU组总Ig E水平高于ASU组,且CSU组和ASU组均高于对照组(P<0.05);CSU组C3水平低于ASU组及对照组(P<0.05);CSU组C4低于对照组,差异有统计学意义(P<0.05);ASU、CSU组IL-2、IFN-γ均低于对照组,差异有统计学意义(P<0.05);CSU组IL-4水平均高于对照组,差异有统计学意义(P<0.05);血清总Ig E水平≥175 mg/L,其与ASU、CSU病情轻、中、重分级呈正相关(P<0.05);血清补体C3、C4水平与CSU病情分级均呈负相关(均P<0.05)。结论 ASU、CSU组各检测结果存在不同,CSU和ASU发病和病情与血清总Ig E和C3、C4相关,而SU发病可能更归因于Th1/Th2相关细胞因子失衡。 Objective To detect the serum total Ig E, complements and correlation of Th1/Th2 cytokines in spontaneous urticaria patients. Methods The serum levels of the total serum Ig E, IL-2, IL-4, IFN-γ were determined by sandwich enzyme linked immunosorbent assay(ELISA), and C3, C4 were determined by immune turbidimetry in 80 patients with acute spontaneous urticaria(ASU), 170 patients with chronic spontaneous urticaria(CSU) and 38 healthy controls. The relationship between the level and the urticaria severity was compared using linear correlation. Results There was significant difference in the level of the total Ig E in three groups(P〈0.05). The level of C3 in patients with CSU was lower than other groups(P〈0.05). The level of C4 in patients with CSU was lower than healthy controls(P〈0.05). The levels of IL-2, IFN-γ were lower in patients with ASU and CSU than healthy controls(P〈0.05). The level of IL-4 was higher in patients with CSU than healthy controls(P〈0.05). The serum total Ig E was positive correlated with the disease classification, when it was more than 175 mg/L(P〈0.05). The level of C3, C4 were negative correlated with the disease classification in the patients with CSU(P〈0.05). Conclusion The results of detection in patients with ASU, CSU are different, which indicates that there are abnormals in the humoral immunity and cell immunity in the patients with SU. The type I allergy may paly more important role in pathogeny of ACU, and the imbalance of Th1/Th2 may induce CSU.
