联合检测多种生化指标对2型糖尿病早期肾损伤的临床诊断价值分析

Joint detection of multiple biochemical indicator on the clinical diagnosis of early renal damage in type 2 diabetes mellitus

目的探讨联合检测多种生化指标对2型糖尿病早期肾损伤的诊断价值。方法研究对象选取我院2014年1月~2015年6月192例2型糖尿病患者,其中尿微量白蛋白阴性的患者为60例设为阴性组,尿微量白蛋白阳性患者71例设为A组,临床尿蛋白阳性患者61例设为B组,同期健康体检者40例作为对照组。对四组受检者的一般资料、肾功能指标、血清胱抑素C（Cys C）、血超敏C反应蛋白（hs-CRP）、同型半胱氨酸（Hcy）、尿微量白蛋白（m ALB）等进行测定对比。结果三组糖尿病患者组血清Cys C、hs-CRP、Hcy水平均较对照组明显升高（P〈0.05）、且A、B组患者较阴性组血清Cys C、hs-CRP、Hcy水平显著升高（P〈0.05）,且A、B组中三者水平与m ALB正相关,随糖尿病肾病的严重程度而升高。三项联合检查的阳性率较各项单独检测的阳性率高,差异有统计学意义（P〈0.05）。结论 Cys C、hs-CRP、Hcy水平与2型糖尿病患者早期肾损伤程度密切相关,其联合检测对于糖尿病肾病的早期诊断、早期治疗及监测有重要的意义。

Objective To investigate joint detection of multiple biochemical indicator on the clinical diagnosis of early renal damage in type 2 diabetes mellitus. Methods 192 cases of type 2 diabetic patients from January 2014 to June 2015 were selected,including 60 cases which Microalbuminuria negative in negative group,71 cases which Microalbuminuria positive as group A,61 cases which urine proteins positive as group B,40 cases of healthy people were set as control group.The physical data,renal function,Cys C,hs-CRP,Hcy,m ALB levels on three groups were measured for comparison. Results The Cys C、hs-CRP、Hcy level of DM patients in three groups were signifintly higher than those in control group（P〈0.05）;The levels of Cys C、hs-CRP、Hcy in group A and B were higher than negative group significantly（P〈0.05）,and its＇ level was positively related to m ALB,The positive rate of combined detection was significantly higher than the positive rate of detection of the individual, the difference was statistically significant（P〈0.05）. Conclusion Cys C、hs-CRP、Hcy level are closely related to patients with type 2 diabetes mellitus early renal damage degree,joint detection of multiple biochemical indicator on the clinical early diagnosis,early treatment and monitoring of early renal damage in type 2 diabetes mellitus is of great significance.
