地佐辛与布托啡诺联合丙泊酚用于无痛人工流产的比较

Comparion of dezocine and butorphanol combined with propofol on painless artificial abortion

目的观察并比较地佐辛与布托啡诺分别联合丙泊酚用于无痛人工流产的镇痛效果﹑苏醒质量及不良反应。方法选择200例早孕未育拟行无痛人工流产术的患者,随机分为A、B两组,每组100例。A组静脉注射地佐辛80μg/kg,B组静脉注射布托啡诺15μg/kg,两组均于5min后静脉注射丙泊酚2.5mg/kg。术中患者体动影响手术操作时酌情追加丙泊酚30~40mg。记录A、B两组手术全程丙泊酚用量,血压、心率,手术操作用时,患者苏醒和定向力恢复时间,离院时间,术后宫缩痛及恶心呕吐眩晕等不良反应情况,评判术中术后镇痛效果。结果两组患者在手术时间﹑丙泊酚用药总量、呼吸循环变化、术毕唤醒时间﹑恶心呕吐的发生率等方面差异无统计学意义。定向力恢复时间、离院时间,A组明显短于B组（P〈0.05）;术后宫缩痛、眩晕发生率A组明显小于B组（P〈0.05）。结论地佐辛与布托啡诺分别联合丙泊酚用于无痛人工流产均能取得良好的麻醉效果,地佐辛联合丙泊酚静脉麻醉,术后患者定向力恢复快,离院时间缩短,有效减轻术后宫缩痛,眩晕发生率低,苏醒质量更高。

Objective To compare the analgesic effect,recovery quality and adverse drug reactions of dezocine and butorphanol combined with propofol in patients undergoing induced abortion. Methods 200 early pregnancy patients for painless artificial abortion were randomly divided into 2 groups,group A and group B,each has 100 cases.A was injected with dezocine 80μg/kg,and B with butorphanol 15μg/kg,two groups were both injected with propofol 2.5mg/kg.Propofol was added 30-40 mg appropriately if body movement influenced the operation.Recorded the total dosage of propofol,circulatory and respiratory changes,operating time,wake-up time,recovery time of orientation,hospital stay time,postoperative uterine contraction pains,and incidence of nausea,vomiting,dizziness.Evaluated the analgesic efficacy in operation and after operation of each group. Results Two groups had no significant differences in operating time,total dosage of propofol,circulatory and respiratory changes,wake-up time,and the incidence of nausea and vomiting.Compared with group B,group A was apparently shorter in recovery time of orientation and hospital stay time（P〈0.05）;and was obviously better in reducing postoperative uterine contraction pains and incidence of dizziness（P〈0.05）. Conclusion Dezocine and butorphanol combined with propofol in patients undergoing induced abortion both can obtain good anesthetic effect.Dezocine propofol combined with intravenous anesthesia is much better on account of faster recovery of orientation,shorter hospital stay time,lower incidence of dizziness,lighter postoperative uterine contraction pains,and higher recovery quality.
