摘要
目的对比分析民营和公立医院的收支状况。方法采集2010~2012年山东省民营医院收支状况普查数据和《山东卫生统计年鉴》公立医院收支数据进行对比分析。结果民营医院3年收入年均增长率为17.24%,支出年均增长率为16.95%,2011、2012年院均年收入环比增长率分别比公立医院低1.26%和11.05%。与公立医院相比,手术收入占医疗收入的比重高5.26%,检查和药品收入分别低2.39%和3.79%。基本药物支出3年间增幅超过5%,药品收支结余下降0.73%。财政补助收入仅为公立医院的0.36%,院均年财政补助收支结余2011年为-0.05万元。结论山东省民营医院平均年收支正逐年增长,但年均增长率低于公立医院。医疗收入中手术收入占比高于公立医院,而检查和药品收入比重低于公立医院。院年均基本药物支出增长快速,对参与政府购买医疗卫生服务有着非常高的积极性。提升规模运营效益,加强医疗安全管理,落实政府购买服务,支持重点学科建设可促进民营医院进一步发展。
Objective To analyze the revenue and expenditure situation and structure of private hospitals. Methods The revenue and expenditure of private and public hospitals from 2010 to 2012 were compared. The data of private hospitals were from a complete survey in Shandong Province, and the data of public hospitals were from Shandong Health Statistical Yearbook. Results The average growth rate per annum of revenue and expenditure were 17.24% and 16.95% respectively. Compared with public hospitals, the annual growth rate of revenue of private hospitals were lower, the proportion of surgery income was higher by 5.26%, and the proportion of medical examination income and medicine income were lower by 2.39% and 3.79% respectively. The proportion of national essential drugs expenditure in medicine expenditure was increased more than 5% from 2010 to 2012. The grant for government was very low. Conclusion The revenue and expenditure of private hospitals were on the rise, however the growth rate were lower than public hospitals. The proportion of surgery income was higher and those of the medical examination as well as medicine were lower. There was high enthusiasm in private hospitals on national essential drugs and basic health care. Increasing the scale benefit, strengthening medical safety management, implementing government purchased services, supporting discipline construction could promote the development of private hospitals.
